摘要 目的观察CAD辅助设计个性化非限制性肿瘤型全膝假体治疗股骨下段肿瘤的临床疗效,探讨其临床价值。方法选择骨科自2012年1月—2014年12月收治的13例股骨下段肿瘤患者,随机分为两组,观察组6例采用CAD辅助设计和个性化非限制性肿瘤型全膝假体重建术,对照组7例应用常规手术治疗。对两组术后1年复发率、膝关节功能及并发症进行比较。结果两组术中手术出血量、1年后复发率、肿瘤瘤段切除长度均无统计学差异(P>0.05)。观察组和对照组术后膝关节MSTS功能评分比较及一年后膝关节活动度比较有统计学差异(P<0.05)。观察组并发症的发生率为16.67%,对照组为42.86%,有统计学差异(P<0.05)。结论 CAD辅助设计个性化非限制性肿瘤型全膝假体治疗股骨下段恶性肿瘤,并发症发生率低,能提高患者关节功能,值得进一步推广研究。 Objective To observe the clinical effect of CAD - assisted design of personalized non - restrictive tumor prosthesis in the treatment of lower femoral tumor and to explore its clinical value. Methods 13 patients with subendrenal tumor were randomly divided into two groups. Six patients in the experimental group were treated with CAD - assisted design and personalized non - restrictive tumor prosthesis reconstruction. 7 patients in the control group were treated with routine surgery. The recurrence rate, knee function and complication were compared between the two groups. Results There was no significant difference in the rate of surgical bleeding, the recurrence rate after 1 year and the length of tumor resection (P 〉 0. 05 ) . There were significant differences in the functional scores of knee joint between the experimental group and the control group (P 〈 0. 05 ) . The incidence of complications in the trial group was 18. 18% and that in the control group was 27. 27% , and there was no significant difference (P 〈 0. 05 ) . Conclusion CAD - assisted design of personalized non - restrictive tumor - type total knee prosthesis in the treatment of lower femoral malignancies, and the incidence of complications is low, can improve the patient's joint function. It is worth further research.
