双套结在桡骨远端骨折小夹板外固定中的应用

The Applied Research of the Clove Hitch in Small Splint External Fixing the Distal Radius Fractures

目的评价采用双套结打结法配合小夹板外固定治疗桡骨远端Colles骨折的临床疗效。方法 136例桡骨远端Colles骨折随机分成2组,治疗组(70例)采用双套结打结法配合小夹板外固定治疗桡骨远端骨折,对照组(66例)采用传统打结方法固定小夹板,定期复查随访,记录观察指标进行评价。结果 136例患者均获得随访,随访12~15 w,平均13.5 w。腕关节功能按照Jakim桡骨远端colles骨折评价标准,第6周治疗组的Jakim评分、优良率均优于对照组(P<0.05),第12周两组间的Jakim评分以及优良率比较无统计学差异(P>0.05)。治疗组外固定所需操作时间明显少于对照组(P<0.05)。结论双套结固定小夹板的外固定方法操作简单,所需时间短,不易松动,患者痛苦小,在早期对维持骨折端的稳定较好。

Objective To evaluate the clinical results of that Clove Hitch tied method with the small splint external fixation in the treatment of comminuted Colles fracture. Methods 136 cases distal radius Colles fracture were divided into 2 groups randomly, the treatment group （70 cases） with Clove Hitch tied method with small splint external fixation for the treat-ment of distal radius fractures, while the control group （66 cases） with traditional banding method, follow - up everyone at the Planning time and record outcome measures to evaluate. Results 136 patients were followed up for 1 2 - 1 5 weeks, aver-age of 13.5 weeks. Wrist joint function according to the evaluation standards of Jakim of Col let fracture of the distal part of the radius, Treatment group^ Jakim score and excellent rates be better than the control group （P 〈 0 . 0 5 ） at the 6 weeks. There is have no difference between the two groups （P 〉 0. 05 ） at the 8 weeks. Operating time needed for external fixation treatment group was obviously less than the control group （ P 〈 0. 05 ） . Conclusion The method of Clove Hitch fixing small splint external is simple in operation, short in time consumption, not easy to loose, less pain for patients, better maintain sta-bility of the fracture at the early period.
