不同组别慢性阻塞性肺疾病急性加重期患者皮质醇水平变化分析

Changes of cortisol levels in patients of different groups of chronic obstructive pulmonary disease with acute exacerbation

目的:通过检测不同组别慢性阻塞性肺疾病急性加重期(AECOPD)患者血液中皮质醇(COR)的水平,探讨AECOPD患者的肾上腺皮质功能状态。方法:选取AEC0PD患者共94例,根据GOLD2011版的标准分为A、B、C、D组,分别为13例、16例、27例、38例,所有患者均在入院后第2天0点(0 AM)、上午8点(8 AM)及下午4点(4 PM)3个时间点分别抽取外周静脉血检测血浆COR。结果:94例AECOPD患者血COR水平增高、正常、降低者分别为41例、46例、7例,其中大于COR 2倍正常值者10例;93例存在COR分泌节律紊乱。4个组的COR水平比较无统计学意义。结论:AECOPD患者存在肾上腺皮质分泌功能紊乱,其血COR水平大部分正常或增高;AECOPD患者血COR水平与病情轻重无明显相关。

Objective： To detect the blood cortisol（COR） levels in patients of different groups of chronic obstructive pulmonary disease with acute exacerbation（AECOPD）,and explore adrenal function in patients with AECOPD. Methods： 94 cases of AECOPD patients,according to the GOLD2011 version of the standard,divided into A,B,C,D groups were 13 cases,16 cases,27 cases,38 cases,all patients were detected the blood plasma COR in the hospital after second day 0 AM,8 AM,4 PM. Results： 94 cases of patients with AECOPD COR level increased,normal and decreased respectively in 41 cases,46 cases,7 cases,10 cases of COR which was greater than 2 times higher than normal,93 cases of COR secreting rhythm disturbance. There were no statistically significant differences between four groups of COR levels. Conclusion： AECOPD patients with adrenal cortical secretion disorders,most of the blood COR level is normal or increased,no significant correlation between COR levels and disease severity in patients with AECOPD.
