我国肥胖青少年运动减肥效果的meta分析 被引量:22

摘要 目的:系统评价不同运动处方对肥胖青少年身体形态、运动能力及血脂水平的影响效果,并根据不同情况提供最优运动处方。方法:计算机检索中国学术期刊数据库、万方数据库、中文科技期刊全文数据库,检索期限均从各数据库建库开始至2016年11月,全面收集运动干预肥胖青少年的实验研究,应用Cochrane偏倚风险评估工具、Revman 5.3软件对纳入研究进行方法学质量评价,运用stata12.0软件进行统计分析。结果:纳入33篇研究文献,共1399名肥胖青少年,文献质量为A级的8篇、B级的14篇、C级的11篇。Meta分析结果:肥胖青少年BMI、BF%均有下降,分别下降了0.84个标准差、3.89个百分点;通过回归分析,确定实验周期是引起BMI异质性的来源之一;肥胖青少年腰围、臀围、WHR分别下降了7.02 cm、4.32 cm、0.024个百分点;肺活量和VO2max得到明显提高,提高了291.17 m L、1.21个标准差;肥胖青少年血脂水平得到改善,TC、TG、LDL-C分别下降了0.39 mmol/L、0.24 mmol/L、0.37 mmol/L,HDL-C升高了0.2 mmol/L。结论:运动干预能有效改善肥胖青少年身体形态、运动能力及血脂水平。肥胖少年运动减肥效果好于青年;与臀围相比,腰围下降更加明显。 Purpose : To evaluate the effect of different exercise prescriptions on obese adolescent' s body form, sports abili- ty, and the effect of blood lipid levels systematically, and present optimal exercise prescription according to the different sit- uations of patients. Methods:using computer to retrieve CNKI, Wan-Fang, VIP database, all the retrieval time was from the start for building a database to Nov. 2016. Exercise intervention in obese adolescent' s experimental researches are compre- hensive collected using the Cochrane bias risk assessment tools to evaluate the methodological quality of included studies, Revman 5.3 software for methodological quality assessment included in the study and having a statistical analysis with sta- tal2.0 software. Results :There are 33 research literature, 1 399 obese adolescent, quality for A grade 8 references, 14 for class B and C grade 11. Meta-analysis showed that obese adolescents' BMI and BF % are decreased by 0.84 percentages and 3.89 standard deviation respectively. Through the regression analysis, the authors determine that the experiment cycle is one of the sources of BMI heterogeneity caused. Obese adolescents waist circumference, hip circumference, WHR fell by 7. 02cm,4.32cm and 4.32 percentage points;Lung capacity and VO2m,x increased by 291.17 mL and 1.21 standard devia- tions significantly;Obese adolescents blood lipid level improved, TC, TG and LDL-C were decreased by 0.39 mmol/L,0. 24 mmol/L and 0.24 mmol/L, and HDL-C increased by 0.2 mmol/L. Conclusions : Exercise intervention can effectively improve the body configuration, obese adolescents exercise capacity and lipid levels. The weight-losing effect for obese ad- olescents is better than that of young people. Compared with hips, waistline is decreased more significantly.
