Refractive criteria of Suresight vision screening instrument in preschool children

目的分析Suresight视力筛查仪对学龄前儿童屈光检查标准与散瞳验光检查符合情况,为学龄前儿童屈光筛查后续处理方案选择提供参考,提高学龄前儿童视力保障水平。方法选取2015年5月-2016年5月在成都市妇女儿童中心医院行儿保检查的360例学龄前儿童为研究对象,共分3个年龄段(6个月~2岁,2~4岁、4~6岁),各120例。所有受检儿童均采用Suresight视力筛查仪检查屈光情况,同时采用1%的阿托品散瞳检影验光,Suresight视力检查仪的屈光判断标准采用Suresight说明书提供的范围进行判定,以1%阿托品散瞳检影验光结果为金标准,计算不同年龄范围儿童Suresight视力筛查仪说明书设置范围检查结果的准确性、特异性和灵敏度,分析Suresight视力筛查仪确定学龄前儿童屈光异常标准的合理性。结果按照Suresight视力检查仪设定的范围,6个月~2岁学龄前儿童Suresight视力检查仪检查的屈光结果特异性为99.55%,灵敏度为62.50%,准确度为97.08%;2~4岁学龄前儿童Suresight视力检查仪检查的屈光结果与散瞳检查结果特异性为98.51%,灵敏度为60.00%,准确度为91.67%,4~6岁学龄前儿童Suresight视力检查仪检查的屈光结果与散瞳检查结果的特异性50.00%,灵敏度为93.81%,准确度为85.41%。6个月~2岁的标准设定一致性和灵敏度均较高,2~4岁和4~6岁设定标准偏高,一致性和灵敏度均较低。结论 Suresight视力筛查仪对筛查学龄前儿童视力方便、快捷,但对异常标准的设定,应根据中国儿童的时机情况予以适当降低,防止漏诊,确保视力出现问题的学龄前儿童均能获得有效的视力保护措施。

Objective To analyze the coincidence of refractive criteria of Suresight vision screening instrument and objective optometry in preschool children,provide reference for selection of processing scheme after refractive screening,and improve the security level of vision in preschool children.Methods A total of 360 preschool children receiving medical examination in the hospital from May 2015 to May 2016 were selected as research objects,then they were divided into three age groups：6 months-2 years,2-4 years,4-6 years,120 children in each age group.Suresight vision screening instrument was used to examine refractive conditions of all the children.Objective optometry using1% atropine was performed,the refractive criteria of Suresight vision screening instrument were determined according to Suresight instruction book,the results of objective optometry using 1% atropine was gold standard,the accuracy,specificity,and sensitivity of refractive criteria of Suresight vision screening instrument among the children in different age groups were calculated.The rationality of ametropia criteria determined by Suresight vision screening instrument in preschool children was analyzed.Results According to Suresight instruction book,the specificity,sensitivity,and accuracy of Suresight vision screening instrument in children aged 6 months-2 years old were 99.55%,62.50%,and 97.08%,respectively;the specificity,sensitivity,and accuracy of Suresight vision screening instrument in children aged 2-4 years old were 98.51%,60.00%,and 91.67%,respectively;the specificity,sensitivity,and accuracy of Suresight vision screening instrument in children aged 4-6 years old were 50.00%,93.81%,and 85.41%,respectively.The consistency and sensitivity in children aged 6 months-2 years old were high,the criteria in children aged 2-4 years old and 4-6 years old were high,and the consistency and sensitivity were low.Conclusion Suresight vision screening instrument is convenient and fast for vision screening in preschool children,but the abnormal standard should be reduced properly based on the practical situations of Chinese children to avoid misdiagnosis and ensure that the preschool children with vision problems can obtain effective vision protection measures.
