白内障超声乳化吸除术联合Ahmed青光眼引流阀植入治疗合并白内障的难治性青光眼的疗效

Effect of Phacoemulsification and Intraocular Lens Implantation Combined with Ahmed Valve in the Treatment of Refractory Glaucoma with Cataract

目的:探讨白内障超声乳化吸除术联合Ahmed青光眼引流阀植入治疗合并白内障的难治性青光眼的疗效。方法:对20例合并白内障的难治性青光眼患者行白内障超声乳化吸除术联合Ahmed青光眼引流阀植入术,术后观察眼压、视力、角膜水肿程度及并发症,随访6个月。结果:20例合并白内障的难治性青光眼中,包括硅油取出术后高眼压(8例),多次滤过手术眼压仍不能控制者(4例),晚期先天性青光眼(3例),葡萄膜炎继发青光眼(5例)。患者术后眼压完全控制15只眼,部分控制2只眼,眼压失控3只眼。手术成功率为85%。结论:白内障超声乳化吸除术联合Ahmed青光眼引流阀植入治疗合并白内障的难治性青光眼是安全的、有效的。

Objective：To evaluate the effect of phacoemulsification and intraocular lens implantation combined with Ahmed valve on the treatment of refractory glaucoma with cataract.Methods：Twenty eyes of 20 patients were treated by phacoemulsification and intraocular lens implantation combined with Ahmed valve implantation.All cases were followed up for 6 months,and IOP control,visual acuity,corneal edema,and complications were observed after the surgery.Results：Among 20 patients with refractory glaucoma combined with cataract,intraocular hypertension post-silicone oil removal（n=8,8eyes）,ocular hypertension patients suffered from several filtering surgery（n=4,4eyes）,late congenital glaucoma（n=3,3eyes）,and glaucoma secondary to uveitis（n=5,5eyes）.After surgery,IOP was well controlled in 15 eyes,partially controlled in 3eyes and uncontrolled in the remaining 2eyes.The success rate of operation was 85%.Conclusion：Phacoemulsification and intralcular lens implantation combined with Ahmed valve in treatment of refractory glaucoma with cataract is safe and effective.
