A/B超引导下激光治疗玻璃体混浊的疗效

Curative Effect of Nd:YAG Laser on Vitreous Floaters Under the Guidance of A/B Ultrasound Scan

目的:观察A/B超联合定位下Nd:YAG激光治疗玻璃体混浊的疗效。方法:2015年1月至2016年3月主诉眼前有黑影飘动的患者177例181只眼纳入治疗组,应用ELLEX公司YAG激光消融机对玻璃体混浊进行激光消融治疗。激光治疗后随访1d、1周、1月、3月,观察患者改善情况,评估患者症状。结果:181只眼中,不同病因所致症状完全消失133只眼(占73.48%),好转48只眼(占26.52%),无效0只眼。其中玻璃体后脱离(PVD)患眼症状完全消失115只眼(占79.86%),好转29只眼(占20.14%)。高度近视患眼症状完全消失8只眼(占29.63%),好转19只眼(占70.37%)。玻璃体液化变性患眼症状完全消失10只眼(占100%)。治疗后随访3月未出现与治疗相关的视力及视野损伤,未出现眼压升高、晶状体损伤,未出现视网膜血管损伤等。结论:A/B超联合定位下Nd:YAG激光治疗玻璃体混浊可以改善视觉质量,是一个安全、操作简便、可重复的治疗方法。

Objective：To observe the effect of Nd：YAG laser on vitreous floaters under the guidance of A/B ultrasound scan.Methods：According to the criteria of the study,one hundred and eighty one eyes of 177 patients with floaters were enrolled.The Nd：YAG laser was used to destroy the vitreous floaters under the guidance of A/B ultrasound scan.Changes of vitreous floaters,visual acuity,IOP,B ultrasound scan results and complications were observed before and after the treatments.Results：There were 133eyes（73.48%）with a complete resolution and 48eyes（26.52%）with an improvement in all patients.Among them,115eyes（79.86%）were completly recovered and 29eyes（20.14%）were improved with posterior vitreous detachment.There were 8eyes（29.63%）with a complete resolution and 19eyes（70.37%）with an improvement in27 eyes with high myopia.Symptoms in ten eyes（100%）with vitreous synchesis and degeneration were completely disappeared.Except that three eyes got a retinal injury including one high myopia,one moderate myopia and one posterior vitreous detachment after trentments,no other complications occured within or after the treatments.Conclusion：The Nd：YAG laser on vitreous floaters under the guidance of A/B ultrasound scan might be a safe and ideal method for treatmentof all persistent vitreous floaters in the future.
