补阳还五汤合四妙散对急性下肢DVT患者凝血及纤溶指标的影响

Effect of Buyang Huanwu Decoction Plus Simiao Powder on the Coagulation and Fibrinolysis Indicators of Patients with Acute DVT in Lower Extremity

目的观察补阳还五汤合四妙散联合尿激酶对急性下肢深静脉血栓(DVT)患者凝血及纤溶指标的影响。方法 40例急性DVT患者随机分为两组各20例,研究组采用补阳还五汤合四妙散联合尿激酶行溶栓抗凝治疗,对照组仅用尿激酶治疗。观察两组的疗效、不良反应,检测两组治疗前后的凝血指标、纤溶酶原活性、D-Dimer水平。结果研究组的治愈率为80.0%,显著高于对照组的50.0%(P<0.05);两组的总有效率比较,差异无统计学意义(P>0.05)。两组治疗前的PT、APTT、TT、FIB比较无统计学差异(P>0.05);在治疗第14天,研究组的PT、APTT、TT均显著长于对照组,FIB显著低于对照组(P<0.05)。两组治疗前的纤溶酶原活性比较无统计学差异(P>0.05);在治疗第14天,研究组的纤溶酶原活性显著低于对照组(P<0.05)。结论补阳还五汤合四妙散联合尿激酶治疗急性DVT疗效显著,通过延长凝血时间、激活纤溶酶原等机制而发挥作用。

Objective To observe the effect of Buyang Huanwu Decoction plus Simiao Powder on the coagulation and fibrinolysis indicators of patients with acute deep vein thrombosis （DVT） in lower extremity. Methods 40 cases of patients with acute DVT in lower extremity were randomly divided into two groups, with 20 cases in each group. The study group was treated with Buyang Huanwu Decoction plus Simiao Powder combined with urokinase, while the control group was treated with urokinase only. The curative effects and adverse reactions of two groups were observed, and the levels of coagulation parameters, plasminogen activity and D-Dimer of two groups were detected. Results The cure rate of study group was 80.0%, significantly higher than 50.0% of control group （P 〈0.05）; No statistical difference was found between two groups in the total effective rate （P〉0.05）. Before treatment, no statistical difference was found between two groups in the PT, APTT, TT and FIB （P 〉0.05）; After treatment, the PT, APTT and TT of study group were significantly longer than those of control group, and the FIB level was significantly lower than that of control group （P 〈0.05）. Before treatment, no statistical difference was found between two groups in the plasminogen activity （P〉0.05）; After treatment, the plasminogen activity of study group was significantly lower than that of control group （P 〈0.05）. Conclusions Buyang Huanwu Decoction plus Simiao Powder combined with urokinase has significant curative effect for the treatment of acute DVT in lower extremity, which plays the role by prolonging the clotting time and activating the plasminogen.
