PFNA与人工股骨头置换治疗复杂型股骨粗隆间骨折的疗效对比

Comparison of curative effect between PFNA and artificial femoral head replacement for the treatment of complex femoral intertrochanteric fracture

目的通过比较PFNA与人工股骨头置换术治疗复杂型股骨粗隆间骨折的疗效,探讨人工股骨头置换术的临床应用价值。方法选取2010年1月至2016年7月来我院接受治疗的复杂型股骨粗隆间骨折患者93例,随机分为置换组与PFNA组,置换组患者应用人工股骨头置换术治疗,PFNA组患者应用PFNA内固定术进行治疗。结果 PFNA组患者的手术耗时与下床时间均长于置换组(P<0.05);PFNA组显效率为56.52%,置换组显效率为85.11%,χ~2=9.219,P=0.002;PFNA组不良反应发生率为21.74%,置换组不良反应发生率为6.38%,χ~2=4.559,P=0.033。结论人工股骨头置换术治疗复杂型股骨粗隆间骨折临床疗效确切,可有效降低手术时间,术后内翻畸形等不良反应发生率低,关节功能恢复好。

Objective： To compare the efficacy of PFNA and artificial femoral head replacement in the treatment of complex femoral intertrochanteric fracture,and to explore the clinical value of artificial femoral head replacement. Methods： 93 patients from January 2010 to July 2016 came to our hospital for treatment of complex femoral intertrochanteric fracture were randomly divided into replacement group and PFNA group,permutation group of patients with artificial femoral head replacement therapy,group PFNA patients were treated with PFNA fixation. Conclusion： the clinical effect of artificial femoral head replacement in the treatment of complex femoral intertrochanteric fracture is accurate,can effectively reduce the operation time,the incidence of adverse reactions such as inversion deformity is low,and the joint function recovery is good.
