曲安奈德联合激光光凝治疗糖尿病黄斑水肿患者疗效及对生活质量影响的观察

Clinical Observation of Efficacy of Triamcinolone Acetonide Combined with Laser Photocoagulation in the Treatment of Diabetic Macular Edema Patients and Their Influence on Life Quality

目的:观察曲安奈德联合激光光凝治疗糖尿病黄斑水肿(DME)的疗效、安全性及对生活质量影响。方法:76例DME患者随机分为对照组和观察组各38例,对照组予激光光凝治疗,观察组在对照组基础上加用曲安奈德玻璃体腔注射。对两组患者最佳矫正视力(BCVA)、黄斑中心凹处视网膜厚度(CMT)、药品不良反应进行观察,采用中文版低视力者生活质量量表(CLVQOL)评价生活质量。结果:治疗后,两组BCVA均较治疗前显著升高,CMT显著降低(P<0.05);且观察组均优于对照组(P<0.05)。治疗后,两组CLVQOL量表各项评分均较治疗前显著升高(P<0.05),且观察组各项评分均明显高于对照组(P<0.05)。两组不良反应发生率相近。结论:曲安奈德联合激光光凝治疗DME优于单用激光治疗,且可提高患者生活质量,值得临床推广。

Objective：To explore the clinical efficacy and safety of triamcinolone acetonide combined with laser photocoagulation for diabetic macular edema （DME） and its impact on quality of life. Methods： 76 patients with DME were randomly divided into control group and observation group, each group had 38 cases. The patients of control group were treated with laser photocoagulation, while the patients of observation group were treated with intravitreal triamcinolone acetonide additionally. The best corrected visual acuity （BCVA）, central macular thickness （CMT） and adverse reactions were observed; the Chinese-version low vision quality of life questionnaire （CLVQOL） was used to evaluate the quality of life. Results： After treatment, the BCVA increased greatly and CMT decreased greatly in both groups （P 〈 0.05 ）, and the BCVA and CMT improved greatly in observation group compared with those in control group （ P 〈 0.05 ）. The CLVQOL scores of two groups were increased significantly after treatment （P 〈 0.05）, and the CLVQOL scores in observation group were significantly higher than those in control＇group （P 〈 0.05 ）. The occurrence rate of adverse reactions was similar. Conclusion： Efficacy of triamcinolone acetonide combined with laser photocoagulation were better than laser photocoagula- tion treatment for patients with DME, and could improve their quality of life, and was worthy of wide clinical use.
