摘要
目的探讨慢性心力衰竭(CHF)病人血浆N端前体脑钠肽(NT-proBNP)及半乳糖凝集素-3(Gal-3)水平的变化及意义。方法选取2015年1月至2016年5月在廊坊市第四人民医院心内科就诊的92例CHF病人为研究对象,所有病人入院后24 h内根据NYHA标准进行心功能分级,并记录年龄、性别和原发病情况;使用超声心动仪测定左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD);分别采用电化学发光法和酶联免疫吸附法测定血浆NT-proBNP及Gal-3水平。结果 (1)不同NYHA分级病人血浆NT-proBNP、Gal-3水平比较差异具有统计学意义(P<0.05),血浆NT-proBNP、Gal-3水平随NYHA分级的升高而升高(P<0.05);(2)LVEF≤40%组血浆NT-proBNP水平高于LVEF>40%组(P<0.05)。LVEDD 50~70 mm组血浆NT-proBNP、Gal-3水平高于LVEDD<50mm组(P<0.05);LVEDD>70 mm组血浆Gal-3水平高于LVEDD<50 mm组及LVEDD 50~70 mm组(P<0.05)。结论血浆NT-proBNP和Gal-3水平与心功能有一定的相关性,其升高程度越大提示心功能越差,可以作为心功能的预测指标。
Objective To explore the change and significance of plasma N-terminal pro-brain natriuretic peptide(NT-proBNP) and galectin-3(Gal-3) levels in patients with chronic heart failure(CHF).Methods Ninety-two cases of patients with CHF were included in this study in the Fourth People's Hospital of Langfang City between January 2015 and May 2016.All patients underwent the cardiac function classification according to NYHA standard within 24 h after admission,and age,gender,primary disease were recorded.Left ventricular ejection fraction(LVEF) and left ventricular end-diastolic diameter(LVEDD) were measured by the ultrasound cardiograph,and the plasma levels of NT-proBNP and Gal-3 were determined by the electrochemical luminescence method and enzyme-linked immunosorbent method respectively.Results(1) The plasma levels of NT-proBNP and Gal-3 in patients with different NYHA classification showed statistically significant differences(P〈0.05).(2) The plasma level of NT-proBNP in patients with LVEF≤40% was higher than that in patients with LVEF 40%.The plasma levels of NT-proBNP and Gal-3 in patients with LVEDD 50-70 mm were higher than those in patients with LVEDD50 mm(P〈0.05) group.The plasma levels of Gal-3 in patients with LVEDD70 mm was higher than that in patients with LVEDD50 mm and LVEDD 50-70 mm(P〈0.05).Conclusions Plasma levels of NT-proBNP and Gal-3 are correlated with cardiac function,which significantly increase with the rise of NYHA classification.NT-proBNP and Gal-3 can be used as the predictors of cardiac function.
