甲状旁腺囊肿47例临床分析

Clinical analysis of parathyroid cysts in 47 cases

目的总结甲状旁腺囊肿的临床及病理特点。方法回顾性分析吉林大学第一临床医院2010至2016年47例甲状旁腺囊肿病例。结果47例患者男9例（19．1％）．女38例（80．9％）。47例中16例（34．0％）行单纯甲状旁腺囊肿切除，17例（36．2％）行囊肿切除＋甲状腺部分切除／次全切除术，14例（29．8％）行囊肿切除＋甲状腺癌扩大根治术／扩大根治及颈部淋巴结清扫术。31例同时行甲状腺手术的患者中，甲状腺病理为结节性甲状腺肿7例，桥本甲状腺炎1例，结节性甲状腺肿合并桥本甲状腺炎6例，甲状腺乳头状癌17例。12例患者术后获得随访，10例行彩超检查无复发，5例行PTH测定及血清钙测定均正常。结论甲状旁腺囊肿临床罕见，少数可能有内分泌功能，对伴随的甲状腺疾病应一并手术处理，确诊需术后病理学支持。

Objective To retrospectively analyze the clinical and pathological features of parathyroid cyst. Methods 47 parathyroid cyst cases admitted and operated on the First Hospital of Jilin University from 2010 to 2016 were retrospectively analyzed. Results Of the 47 patients, 9 cases were male （19. 1% ） and 38 cases were female （80. 9% ）. 16 cases （34%） underwent simple cyst excision, 17 cases （36. 2% ） did cyst excision and partial/total thyroidectomy, 14 cases （29. 8% ） did cystectomy and radical thyroidectomy/lateral neck dissection. Overall 31 patients undergoing concurrent thyroid surgery there were pathologically identified 7 cases of nodular goiter（22. 6% ） , 1 case of Hashimoto＇s thyroiditis （3.2%）, 6 cases of nodular goiter and Hashimoto＇s thyroiditis （ 19. 4% ）, 17 cases of papillary thyroid carcinoma （54. 8% ）. 12 patients were postoperatively followed-up, with 10 being Doppler ultrasound followed without recurrence and 5 reexamined parathyroid hormone and serum calcium within normal range. Conclusions Parathyroid cyst is a rare clinical disease, most are non-functional,diagnosis relies heavily on pathological examination.
