Viabahn覆膜支架治疗下肢动脉硬化闭塞症中短期疗效观察

Short Term Efficacy of Viabahn Covered Stent in the Treatment of Lower Extremity Arteriosclerosis Obliterans

目的:利用临床病例对患有下肢动脉硬化闭塞症患者在Viabahn覆膜支架治疗下的中短期疗效进行探究。方法:选取本院2013年12月-2015年12月进行下肢动脉硬化闭塞症治疗的84例患者,采用随机数字表分为试验组和对照组,每组42例。其中对照组采用裸支架疗法,而试验组采用Viabahn覆膜支架疗法,随后对所有患者治疗后的中短期疗效进行比较分析。结果:试验组患者手术后3、6、9、12个月时的通畅率别为95.2%、85.7%、83.3%、74.0%,手术后12个月ABI指数为(0.82±0.19)。而对照组患者手术后3、6、9、12个月时的通畅度分别为90.5%、76.2%、2.0%、52.4%,手术后12个月ABI指数为(0.78±0.21);两组患者手术后3、6个月时的通畅率比较,差异均无统计学意义(P>0.05),试验组患者9个月和12个月时的通畅率高于对照组,差异均有统计学意义(P<0.05);两组ABI指数比较,差异无统计学意义(P>0.05)。结论:对患有下肢动脉硬化闭塞症患者采取Viabahn覆膜支架治疗,可提高患者的治疗效果,该法可作为治疗近关节下肢动脉硬化闭塞症,尤其是跨膝关节病变的首选方法。

Objective： To study the short term efficacy of lower extremity arterial occlusive disease treated with Viabahn covered stent treatment.Method： From December 2013 to December 2015, 84 lower extremity arteriosclerosis obliterans patients treated in our hospital were selected and divided into experimental groyp and control group according to the random number table method, 42 patients in each group.The control group was adopted bare stent therapy, while the experimental group adopted Viabahn stent therapy, the short-term curative effect of all patients after treatment were compared.Result： 3, 6, 9, 12 months after surgery, the pateney rate of the experimental group were 95.2%, 85.7%, 83.3%, 74.0% respectively, ABI index was （ 0.82 ±0.19 ）, and the control group＇s pateney rate in 3, 6, 9, 12 months after surgery were 90.5%, 76.2%, 2.0% and 52.4% respectively, and ABI index was （ 0.78 ±0.21 ）, the patency rate in 3 and 6 months after operation of the two groups were no significant difference （ P〉0.05 ）, while in the 9 and 12 months after surgery, the patency rate of the experimental group were higher than that of the control group, the differences were statistically significant （ P〈0.05 ） .ABI index between two groups were no significant difference （ P〉0.05 ） .Conclusion： Patients with lower limb arteriosclerosis obliterans treated with Viabahn stent, can improve the treatment effect, the method can be first choice of lower extremity arterial occlusive disease, especially for cross knee lesions.
