我国碎片化的老年照料体系难以满足老年人多元、连续和完整的照料需求,也造成了医疗与社会照料资源的浪费。这种内在张力迫切要求将碎片化了的老年照料体系整合起来,而西方发达国家正在倡导的整合照料理念对我国老年人照料体系的整合优化有重要借鉴意义。本文从整合照料的内涵、兴起的背景出发,介绍了整合照料的原则、多重路径、典型模式、争论与反思等,全面分析了西方整合照料的理念与实践。并在归纳西方整合照料成功经验基础上,对我国老年照料政策的整合优化提出了建议。

The fragmentation of the elderly care system in China has been hard to meet the multivariate, continuous and complete needs of the elderly, and it also caused the waste of the health and social care resources. The inner tension urgently needs to integrate the fragmented elderly care system, while the integrated care which the western countries are advocating has its important significance to the integrated optimization of the elderly care in our country. The paper begins from the connotation of the integrated care, the background of the emergence, principle, approach and the debate introspection aroused and the typical integrated care mod- el, making a comprehensive introduction and analysis to the concept and practice of the integrated care in western countries. On the basis, the paper extracts the lessons learned from the success of the western integrated care, and put forward the suggestions to the integrated optimization of the elderly care policies in China.
