精神科外出会诊状况分析

Analysis of the status of going out for consultation of psychiatric department

目的:了解精神科外出会诊状况,规范联络会诊流程,提高转诊及诊治效率。方法:收集2012年~2015年精神科到综合医院外出会诊病例315例,并进行回顾性分析。结果:精神科外出会诊以公立三甲综合医院为主(70%),并呈逐年上升趋势;请会诊科室以外科(24.4%)神经内科(24.1%)心内科(15.6%)居多,会诊患者涉及多系统疾病;会诊诊断以器质性精神障碍居多(65%),多以抗焦虑、抗抑郁剂及抗精神病药物治疗,部分患者予以心理治疗或转入精神科医院住院治疗。结论:综合医院对精神科疾病的认识普遍提高,会诊患者病情复杂,病种繁多。精神科联络会诊有助于满足综合医院住院患者的精神卫生服务需求,规范、高效完成患者的转诊转院,有利于疾病的及时诊断与治疗。

Objective： To understand the situation of psychiatric going out for consultation,standardize the consultation process,and improve the efficiency of referral and treatment. Methods： 315 cases from 2012 to 2015,which needed the consultation of the general hospital from the psychiatric department,were collected and analyzed retrospectively. Results： The going out for consultation of the psychiatric department was mainly from the public top three comprehensive hospitals（ 70%）,and increased year by year.The consultation departments were surgical department（ 24. 4%）,neurology department（ 24. 1%）,and cardiology department（ 15.6%）. The multiple system diseases were involved,among which,the organic mental disorders were the most（ 65%） and treated with anti-anxiety,antidepressant and antipsychotic drugs. Some patients needed the psychological treatment or to be hospitalized. Conclusions： The general hospital has an improved awareness of the psychiatric disorders and can be consulted with wide variety of diseases. The consultation liaison psychiatry could help meet the needs of the inpatients in the general hospital mental health services,and standardize and efficiently complete the patient referral,and is beneficial to timely diagnosis and treatment of the disease.
