摘要 目的 :探讨甲状腺影像和数据报告系统(TI-RADS)对甲状腺微小癌的诊断价值。方法 :回顾性分析188例共195个甲状腺微小结节(直径≤10 mm)完整超声检查资料,并进行TI-RADS分类。以病理结果作为金标准,计算超声TI-RADS诊断甲状腺微小癌的敏感度、特异度、阳性预测值和阴性预测值,绘制超声TI-RADS诊断甲状腺微小癌ROC曲线,计算ROC曲线下面积。结果:TI-RADS诊断甲状腺微小癌的灵敏度、特异度、阳性预测值、阴性预测值及ROC曲线下面积分别为98.8%、35.5%、54.2%、97.5%、0.764。结论 :超声TI-RADS分类方法对甲状腺微小癌具有较高的诊断价值。 Objective:To investigate the value of ultrasound thyroid imaging reporting and data system (TI-RADS) in the diag-nosis of thyroid microcarcinoma (TMC). Methods:Integrated ultrasonic data of 188 patients with 195 thyroid micro nodules (maximum diameter≤10 mm) were analyzed retrospectively. The pathology results were regarded as the gold standard to calcu-late the sensitivity,specificity,positive predictive value and negative predictive value of ultrasound TI-RADS in the diagnosis of TMC. The receiver operating characteristic (ROC) curve of ultrasound TI-RADS were drawn. The area under the ROC curve were calculated. Results:The sensitivity,specificity,positive predictive value and negative predictive value of ultrasound TI-RADS in the diagnosis of TMC were 98.8%、35.5%、54.2%、97.5%,respectively. The area under the ROC curve were 0.764. Conclu-sions:The ultrasound TI-RADS are effective diagnostic procedures for TMC.
