青年和中老年胰腺实性假乳头状瘤患者CT表现对比研究

Comparative study of the CT features between Young,middle-aged and elderly patients with Solid pseudopapillary tumour of pancreas

目的 :通过分析青年和中老年胰腺实性假乳头状瘤(SPTP)患者的CT特征,提高放射科医师对SPTP CT特征的认识。方法:回顾性分析29例(中老年11例,青年18例)SPTP的CT平扫及增强扫描表现。观察病变的位置、形态、边界、强化情况等CT特征。采用χ~2检验、Fisher精确检验及t检验进行统计学分析。结果:29例均为单发。中老年组肿瘤边界清晰5例,模糊6例;青年组肿瘤边界清晰15例,模糊3例;2组差异有统计学意义(P<0.05)。中老年组肿瘤实性部分增强扫描后动脉期、实质期CT值分别为(57.6±14.9)、(58.8±12.0)HU,囊性部分CT值为(21.5±5.0)HU;青年组肿瘤实性部分增强扫描后动脉期、实质期CT值分别为(62.6±10.6)、(69.1±11.3)HU,囊性部分CT值为(24.0±4.6)HU。其中实性部分实质期CT值2组差异有统计学意义(P<0.05)。结论:中老年SPTP与青年患者相比肿瘤实质期CT值更低,更易出现边界模糊。

Objective：To improve the radiologists＆#39; awareness of the CT features of solid pseudopapillary tumour of pancreas through the analysis of the CT features between young,middle-aged and elderly patients with solid pseudopapillary tumour of pancreas. Method：The findings of unenhanced and contrast-enhanced CT scans of 29 cases （11cases of middle-aged and el-derly,18 cases of young） with solid pseudopapillary tumour of pancreas were retrospectively analyzed. CT characteristics of the position,shape,boundary,enhancement of the lesion were observed. The χ2 test,Fisher exact test and t test were used for statis-tical analysis. Results：All cases were solitary lesion,with no distant metastasis. Boundary conditions：In the middle and old aged group,the tumor boundary was clear in 5 cases and fuzzy in 6 cases. In the youth group,the tumor boundary was clear in 16 cases,and fuzzy in 2 cases. The difference was statistically significant （P〈0.05）. Comparison of CT values after tumor enhance-ment：In the middle and old aged group,the CT values in the arterial phase and the parenchymal phase after the solid part of the tumor was enhanced were （57.6±14.9） HU and （58.8±12.0） HU respectively;Cystic part CT was （21.5＋5.0） Hu. In the youth group,the CT values in the arterial phase and the parenchymal phase after the solid part of the tumor was enhanced were （62.6±10.6） HU and （69.1±11.3） HU respectively;Cystic part CT was （24.0±4.6） HU. There were statistically signifi-cant differences in the parenchymal phase CT value of the solid part （P〈0.05）. Conclusion：Compared with the young pa-tients of solid pseudopapillary tumour of pancreas,the parenchymal phase CT value of the solid part is lower and they are more prone to boundary fuzzy in the middle-aged and elderly patients.
