院外持性血液透析后并发脑出血脑疝形成患者的急诊护理干预

Emergency nursing intervention for patients with cerebral hemorrhage cerebral hernia after maintenance hemodialysis outside hospital

目的:探讨院外持性血液透析后并发脑出血脑疝形成的急诊护理措施干预。方法:选择2014年1月~2015年12月我院收治的行血液透析合并脑出血脑疝形成者40例为研究对象,统计患者确诊时间、入手术室时间,出现护理并发症及抢救成功例数、死亡率。结果:患者确诊时间为(13.5±3.6)min、入手术室时间为(23.5±5.8)min;出现未及时建立静脉通道者1例(2.5%)、发生骶尾部发红甚至溃疡者1例(2.5%),未进行头部及时配皮者1例(2.5%);抢救成功例数为36例(90.0%),死亡例数为4例(10.0%)。结论:对于院外持性血液透析后并发脑出血脑疝形成的进行有效的急诊护理,能显著缩短确诊时间和入手术室时间,减少护理并发症,提高抢救成功率。

Objective To discuss the emergency nursing intervention for cerebral hemorrhage cerebral hernia after maintenance hemodialysis outside hospi-tal. Methods ： Selected 40 patients receiving maintenance hemodialysis with cerebral hemorrhage cerebral hernia admitted in our hospital from January 2014 to December 2015 as the research object, and the patients were counted in diagnosis time, operation room entry time, number of nursing complications and number of successful rescue and death rate. Results： The diagnosis time of the patients was （13 ？ 5 ± 3 ？ 6） min, operation room entry time was （23 ？ 5 土 5.8） min; 1 patient had no establishment of vein passage timely （2. 5% ）,1 patient had sacrococcygeal region redness and even ulcer （2. 5% ）,1 patient did not have scalp matching timely （2. 5% ） ;36 patients were rescued successfully （90. 0% ） and 4 patients died （ 10 .0% ） . Conclusion： The effective e-mergencynursing for the patients with cerebral hemorrhage cerebral hernia after maintenance hemodialysis outside hospital could significantly reduce the di-agnosis time and operation room entry time, reduce the nursing complications and increase the successful rate of rescue.
