摘要
目的探讨T2DM患者血清Irisin水平与脂肪细胞型脂肪酸结合蛋白(FABP4)的相关性。方法招募上海市奉贤区志愿者460名,根据75 g OGTT结果确定新诊断T2DM患者(T2DM组)56例,糖耐量正常者112名作为对照组(NGT)。测定糖脂代谢参数、Irisin及FABP4水平。结果 T2DM组WC、BMI、体脂百分比、SBP、DBP、ALT、TG、FPG、2 hPG、HbA_1c、HOMA-IR、SUA、eGFR较NGT组升高,HOMA-β低于NGT组(P<0.05或P<0.01)。Pearson相关性分析显示,Irisin与eGFR呈正相关;T2DM组血清Irisin与HbA_1c呈正相关(r=0.292,P=0.029),与FABP4呈负相关(r=-0.285,P=0.033)。校正年龄、BMI、体脂百分比、SBP、TC、eGFR,行多元线性回归分析结果显示,T2DM组HbA_1c和FABP4是Irisin的独立影响因素(R^2=0.350,P=0.025)。结论本研究表明,Irisin水平与糖脂代谢及肾功能密切相关。新诊断T2DM患者血清Irisin与FABP4呈负相关,提示Irisin可能有拮抗FABP4的作用。
Objective To explore the relationship between serum Irisin level and adipocyte fatty acid binding protein (FABP4) in patients with type 2 diabetes mellitus (T2DM). Methods A total of 460 volunteers were recruited from Fengxian District,Shanghai.75 g OGTT was performed in all the subjects.According to the results,two study groups were established:patients with newly diagnosed diabetes mellitus (T2DM group,n=56),and patients with normal glucose tolerance (NGT group,n=112).Indexes of glucose and lipid metabolism,Irisin and FABP4 were measured. Results WC,BMI,body fat percentage,SBP,DBP,ALT,TG,FPG,2 hPG,HbA1c,HOMA-IR,SUA,and eGFR were significantly higher,and HOMA-β was significantly lower in T2DM group than in NGT group (P〈0.05).Pearson correlation analysis showed that Irisin was positively related to eGFR in both groups;and also positively related to HbA1c (r=0.292,P=0.029),negatively related to FABP4 (r=-0.285,P=0.033) in T2DM group.Multivariate linear regression analysis showed that HbA1c and FABP4 are independent risk factors for Irisin (R2=0.350,P=0.025) after adjusting for age,body fat percentage,SBP,TC,and glomerular filtration rate (eGFR). Conclusion We found that the level of Irisin is closely related to glucose and lipid metabolism and renal function.Serum Irisin may have an antagonistic effect on FABP4.
