强化去铁治愈重型地中海贫血铁超载并糖尿病一例

Diabetes secondary iron overload in a patient with thalassemia major cured by intensified iron chelate

糖尿病是重型地中海贫血(TM)患者铁超载的重要并发症之一。对1例TM患者在胰岛素基础上联合强化排铁治疗。随着铁蛋白的快速下降,患者胰岛素敏感性增加,胰岛素分泌功能改善,胰岛素用量逐步减少并最终停止使用。TM患者需监测其胰岛功能,强化排铁有望治愈早期发现的糖尿病。

Diabetes mellitus is an important complication secondary to iron overload in patients with thalassemia major. One case was treated with insulin and intensified iron chelation therapy. During treatment,together with the rapidly decreasing of serum ferritin level,insulin sensitivity was increased and insulin secretion function was improved in this patient,therefore insulin dosage was decreased and finally halted. Function of pancreas should be monitored in patients with thalassemia major. Early extensive iron chelation therapy may stop the development of diabetes mellitus.
