GLP-1受体激动剂在2型糖尿病患者中的心血管保护作用及其潜在机制 被引量:6

摘要 心血管疾病(CVD)是2型糖尿病(T2DM)患者最常见的致残和致死原因,其发生风险较非糖尿病人群高2~4倍。胰高糖素样肽1(GLP-1)是一种肠促胰素,其主要生物学作用是参与血糖稳态调节。天然的GLP-1易被二肽基肽酶4(DPP-4)快速灭活,半衰期仅为1~2 min,故其临床应用价值有限。GLP-1受体激动剂(GLP-1RA)因改变了氨基酸序列,故可免受DPP-4的迅速降解。目前在国外上市的GLP-1RA已有多种,短效制剂包括艾塞那肽和利司那肽,长效制剂包括利拉鲁肽、艾塞那肽周制剂、阿必鲁肽、度拉糖肽等。其中艾塞那肽和利拉鲁肽是目前在我国上市的两种GLP-1RA。艾塞那肽是第1个被批准上市的GLP-1RA,与人GLP-1的氨基酸序列只有53%同源性。利拉鲁肽是第2个上市的GLP-1RA,与人GLP-1具有97%同源性。近年的研究显示,GLP-1RA具有心血管系统的保护作用[2]。本文对GLP-1RA的心血管保护作用及其潜在机制进行阐述。
