简易体位辅助经额穿刺引流术治疗高血压性基底节区出血

Simple posture-assisted transfrontal puncture evacuation for hypertensive basal ganglia hemorrhage

目的探讨简易体位辅助经额穿刺引流术治疗高血压性基底节区出血的效果。方法回顾性分析2015年1月至2016年9月采用简易体位辅助下快速经额穿刺引流术治疗的27例高血压性基底节区出血的临床资料。结果术前准备时间25~39min,平均33 min;手术时间33~58 min,平均36 min;穿刺成功率达100%。术后发生再出血2例,行开颅血肿清除+去骨瓣减压术;术后发生应激性溃疡3例、肺部感染5例,对症治疗后改善。术后无颅内感染、癫痫等并发症,术后30 d内无死亡。术后3个月根据日常生活能力评分标准:Ⅰ~Ⅲ级19例,Ⅳ级5例,V级3例。结论简易体位辅助经额穿刺引流术治疗高血压性基底节区出血简单、实用、安全、有效。

Objective To explore the curative effects of simple posture-assisted transfrontal puncture evacuation on the hypertensive basal ganglia hemorrhage. Methods The clinical data of 27 patients with hypertensive basal ganglia intracerebral hemorrhage who were treated by simple posture-assisted transfrontal puncture evacuation from January, 2015 to September, 2016 were analyzed retrospectively. Results The success rate of the puncture was 100%. The preoperative preparation time ranged from 25 to 39 minutes with a mean time of 33 minutes. The operative time ranged from 33 to 58 minutes with a mean time of 36 minutes. Delayed intraeranial hemorrhage oeurred in 2 patients who received craniotomy. No patient died within 30 days after the operation. Activities of＂ daily living index ranged from grades 1 to 3 in 19 patients, was grade 4 in 5 and grade 5 in 3 3 months after the operation. Conclusion The simple posture-assisted transfrontal puncture evacuation is a simple, practical, safe and effective method to treat hypertensive basal ganglia hemorrhage.
