阴虚水鼓方联合西药治疗肝硬化腹水(气阴两虚)随机平行对照研究

Randomized Parallel Controlled Study on the Treatment of Liver Cirrhosis with Ascites(Yin and Yang and Yin and Yang) by Yin Deficiency and Water Drum Combined with Western Medicine

[目的]观察阴虚水鼓方联合西药治疗肝硬化腹水(气阴两虚)疗效。[方法]使用随机平行对照方法,将50例住院患者按病志号抽签简单随机分两组。对照组25例控制水钠摄入及输液量,给予高热量、高蛋白、低脂、低盐、维生素和流质饮食;利尿、每周定期输注白蛋白或血浆、定期排放腹水。治疗组25例阴虚水鼓方(太子参30,麦冬20g,五味子10g,阿胶20g,枸杞30g,鳖甲20g,车前仁40g,白茅根30g,砂仁8g,当归15g,郁金10g),1剂/d,水煎300m L,早晚口服;西药治疗同对照组。连续治疗30d为1疗程。观测临床症状、ALT、AST、TBIL、AIB、复发率、不良反应。治疗1疗程,判定疗效。随访半年,观测有效病例复发情况。[结果]治疗组治愈12例,好转11例,无效2例,总有效率92.00%;对照组治愈9例,好转9例,无效7例,总有效率72.00%;治疗组疗效优于对照组(P<0.05)。ALT、AST、TBIL、AIB改善治疗组优于对照组(P<0.01)。[结论]阴虚水鼓方联合西药治疗肝硬化腹水(气阴两虚),疗效满意,无严重不良反应,值得推广。

[Objective] To observe the therapeutic effect of Yin deficiency and water drum combined with western medicine in treatment of liver cirrhosis with ascites. [Method] Using random parallel control method, according to the records, draw a simple disease were randomly divided into two groups of 50 cases of hospitalized patients. 25 cases of control group control of water and sodium intake and infusion volume, give high calorie, high protein, low fat, low salt, vitamins and diet; diuresis, weekly infusion of albumin or plasma periodic discharge of ascites, 25 cases in the treatment group. Yin deficiency （from 30 party water drum, Ophiopogon japonicus 20g, Schisandra 10g, gelatin 20g, medlar 30g, turtle 20g, Plantago Jen 40g, Rhizoma imperatae 30g, Amomum 8g, angelica 15g, turmeric 10g） 1 agent/d, 300mL oral decoction, western medicine treatment sooner or later; the same as the control group. Continuous treatment with 30d for 1 courses. Observation of clinical symptoms, ALT, AST, TBIL, AIB, the recurrence rate and adverse reaction. To determine the efficacy of 1 courses of treatment, follow-up observation. The first half of the year, effective cases of recurrence. [Results] The treatment group 12 cases were cured, 11 cases improved, 2 cases ineffective, the total efficiency of 92%; control group 9 eases were cured, 9 cases improved, 7 cases ineffective, the total efficiency of 72%; the treatment group than the control group （P〈0.05）. ALT, AST, TBIL, AIB improvement in treatment group than in control group （P〈0.01）. [Conclusion] The treatment of cirrhosis ascites （Yin and Yang deficiency） with Yin deficiency and water drum combined with western medicine has satisfactory curative effect and no serious adverse reactions.
