督灸联合康复训练改善脑卒中恢复期偏瘫下肢功能随机平行对照研究

A Randomized Parallel Control Study on Improving the Function of Lower Limbs in Patients with Hemiplegia after Stroke Recovery with Du Moxibustion

[目的]观察督灸联合康复训练改善脑卒中恢复期偏瘫下肢功能。[方法]使用随机平行对照方法,将80例门诊患者按就诊顺序编号简单随机分两组。对照组40例康复训练,根据患者运动功能,主动性训练为主,上下肢关节屈伸控制与自主性运动、扩大手指伸展运动范围、上下肢负重、坐位起立动作、坐位、立位平衡、下肢关节分离运动、上下阶梯训练以及改善上肢、手协调及精细动作的训练,1次/周,4次/月。治疗组40例督灸:沿督脉循行自C7至S1涂擦鲜姜汁,灸粉(丁香、肉桂、麝香、沉香、干姜、斑蝥、桑枝、伸筋草、附子、桂枝、透骨草、海风藤、草乌,每份2.5g)均匀铺撒,覆桑皮纸、铺放姜泥,修整呈梯形,再置长条形艾炷,连灸3壮,1次/月;康复训练同对照组。治疗1个月为1疗程。观测临床症状、CSS、FMA、BSS、MBI、不良反应。连续治疗2疗程,判定疗效。[结果]CSS、FMA、BSS、MBI改善治疗组优于对照组(P<0.01)。[结论]督灸联合康复训练改善脑卒中恢复期偏瘫下肢功能,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the image of lower limb function in patients with hemiplegia after recovery from stroke by rehabilitation training combined with moxibustion. [Methods] Eighty patients were randomly divided into two groups according to the method of randomized parallel control. 40 eases in the control group, given rehabilitation training, l/d, 6 times/week. 40 eases in the treatment group were treated with moxibustion and moxibustion at the site of the moxibustion （along the line from C7 to S1）. The ginger mud and the moxibustion powder （clove, cinnamon, musk, aloe, ginger, eantharidin, mulberry, Speranskia, sea breeze rattan, Aconitum, each 2.5g）, set long strip moxa cone, moxibustion with 3 strong. （FBS）. The motor function of the lower limbs was assessed by the Fugl-Meyer Rating Scale （FMA）, and the Berg Balance Scale （BBS） was used to evaluate the motor function of the lower limbs. The score of BBS was evaluated by the Spasticity Scale （CSS） Balance function and improved Barthel index （MBI） to assess the ability of daily living activities （ADL） to observe adverse reactions. [Result] The scores of CSS, FMA, BSS and MBI in the two groups were improved （P〈0.05）, and the moxibustion group was better than the control group （P〈0.05）. [Conclusion] Du moxibustion combined with rehabilitation training can effectively improve the ankle spasm of stroke patients with hemiplegia, improve the lower limb movement, balance function and activities of daily life, no serious adverse reactions, worthy of promotion.
