心肌酶测定在柯萨奇病毒感染患儿诊断中的临床意义

Clinical significance of myocardial enzymes in the diagnosis of coxsackie virus infection in children

目的探讨血清AST、LDH、CK、CK-MB、HBDH心肌酶在柯萨奇病毒感染时心肌损害诊断时的变化。方法采用酶速率法检测观察组47例疱疹性咽颊炎患儿和对照组45例普通上呼吸道感染患儿的心肌酶水平。结果观察组心肌酶指标均高于对照组,差异有统计学意义(P<0.01)。47例疱疹性咽颊炎患儿中,心肌酶增高38例(80.82%),正常9例(19.18%);45例对照组的心肌酶增高17例(37.83%),正常28例(62.17%),2组差异有统计学意义(P<0.01)。疱疹性咽颊炎组心电图改变24例,对照组心电图改变8例,观察组明显高于对照组,差异有统计学意义(P<0.01)。结论血清心肌酶水平变化对柯萨奇病毒感染患儿心肌损害的诊断具有重要临床意义,值得重视。

Objective To explore the changes of serum AST,LDH,CK,CK-MB,HBDH myocardial enzymes in the diagnosis of myocardial damage caused by coxsackie virus infection. Methods Enzyme rate method was used to detect the myocardial enzyme levels of 47 cases of herpetic pharynx buccal phlogistic children in the observation group and 45 cases of children with common sense in the control group. Results Myocardial enzyme index of the observation group were higher than the control group,and the difference was statistically significant（ P〈 0. 01）. Among the 47 cases of herpetic pharynx buccal phlogistic children,myocardial enzymes increased in 38 cases（ 80. 82%）,stayed normal in 9 cases（ 19. 18%）; in 45 cases of the control group,myocardial enzymes increased if 17 cases（ 37. 83%）,stayed normal in 28 cases（ 62. 17%）,and the difference between the two groups was statistically significant（ P 〈0. 01）. In the herpetic pharynx buccal group,electrocardiogram changed in 24 cases,electrocardiogram changed in 8 cases of the control group,so the observation group was significantly higher than the control group,and the difference was statistically significant（ P〈 0. 01）. Conclusion The changes of serum myocardial enzyme levels in the diagnosis of myocardial damage in children caused by coxsackie virus infection have important clinical significance,and is worth attention.
