临床药师对子宫内膜癌化疗后并发静脉血栓栓塞老年患者的药学监护

Pharmaceutical care for a gerontal patient with endometrial cancer combined lower extremity deep vein thrombosis after chemotherapy

目的探讨临床药师在子宫内膜癌老年患者并发下肢深静脉血栓临床治疗中的工作切入点。方法临床药师参与患者治疗的全过程,在抗凝药物的选择、药物剂量调整方面向医师提出适当的建议,并密切监测凝血功能改变和药物相互作用,防止不良事件发生。结果临床药师的部分建议被医师采纳,患者顺利完成抗凝治疗,未发生任何严重不良反应。结论临床药师参与该类患者的临床治疗实践,对患者进行药学监护,有利于提高药物治疗的安全性和有效性。

Objective To explore the specific path for clinical pharmacists in treating gerontal patients with endometrial cancer combined lower extremity deep vein thrombosis. Methods Clinical pharmacists participated in treatment by helping with anticoagulant drugs selection and dosage adjustments, as well as monitoring the changes in blood coagulation and drug interactions to prevent adverse events. Results Some recommendations by clinical pharmacists were adopted by doctors, and the anticoagulant therapy was successfully conducted with no serious adverse reactions. Conclusion Clinical pharmacists＇ participating in clinical practice and pharmaceutical care helps increase the efficacy and safety of drug therapy.
