全程化药学服务对口服抗癫痫药患者临床效果及用药依从性的影响

Effect of whole-course pharmacological service on clinical effect and medication compliance in patients taking oral antiepileptic drugs

目的评价全程化药学服务对口服抗癫痫药患者治疗效果及用药依从性的影响。方法 2013年1月-2015年10月经本院神经内科确诊为癫痫的成人患者26例,详细记录患者实施干预前治疗效果及用药依从性,征得患者同意后,临床药师实施全程化药学服务,如预测患者不同给药剂量下药物的稳态血药谷浓度、向患者提供《癫痫用药及健康教育手册》及《癫痫用药日记》、嘱咐复诊并进行血药浓度监测(TDM)、定期(每3个月1次)随访等,1年后记录患者治疗效果及用药依从性。结果临床药师实施全程化药学服务后,患者癫痫发作次数、不良反应发生率明显降低;癫痫用药知识知晓率、用药依从性明显提高,与干预前比较差异有统计学意义(P<0.05)。结论全程化药学服务对提高口服抗癫痫药患者治疗效果及用药依从性有良好的效果。

Objective To determine the influence of whole-course pharmacy service on the therapeutic effect in patients taking oral antiepileptic drugs. Methods Totally 26 adult patients were enrolled in this study who were diagnosed with epilepsy by the Department of Neurology in our hospital from January 2013 to October 2015. Clinical effect and compliance of medication were recorded in details before the pharmacological service. With the approval of the patients, clinical pharmacists began such services as providing epilepsy medication and health education manual and epilepsy medication diary, asking patients for referral and therapeutic drug mon- itoring （TDM）, regular follow-up services （once every 3 months）, etc. Clinical effect and compliance of medi- cation were recorded one year after the service. Results The number of epileptic seizures and the incidence of adverse reactions were significantly lowered after the pharmacological service （P 〈 0.05）. The awareness rate of medication knowledge and compliance of epilepsy drug use were significantly improved and the difference was significant （P 〈 0.05）. Conclusion Whole-course pharmacological service helps improve the therapeutic effect of patients taking oral antiepileptic drugs.
