改良侧卧位在胸腔镜食管癌手术中的应用研究 被引量:7

摘要 目的探讨改良侧卧位在临床胸腔镜食管癌手术中的应用效果。方法选择2015年11月~2016年11月胸腔镜下食管癌手术患者100例,将其随机分为对照组和观察组。对照组实施常规侧卧体位安置,观察组实施改良后的体位安置,比较两组患者术前、术后的血流动力学稳定性、SpO_2及平均气道压力,术后体位并发症与手术医师满意度的情况。结果与对照组相比,观察组患者术前、术后的血流动力学较稳定;观察组体位并发症发生率显著低于对照组(χ~2=7.98,P<0.05);观察组手术医师的满意度显著高于对照组(χ~2=19.64,P<0.05)。结论改良侧卧位可以有效地提高术中患者生命体征的稳定性,降低体位并发症的发生率,增加患者的舒适与安全性,提高手术医师的满意度,满足手术室优质护理的需求。 Objective To investigate the application effect of modified lateral position in the operation of clinical thora- eoseopic esophageal cancer. Methods A total of 100 patients receiving thoracoscopic esophageal cancer surgery from November 2015 to November 2016 were selected. They were randomly divided into control group and observation group. The control group was given conventional lateral position, and the observation group was given the modified po- sition. The hemodynamics stability, SpO2 and mean airway pressure, postoperative postural complications and satisfac- tion of the surgeon were compared between the two groups before and after the surgery. Results Compared with the control group, the hemodynamics of the patients in the observation group were stable before and after operation. The in- cidence rate of postural complications in the observation group was significantly lower than that in the control group (X2=7.98, P〈0.05); the satisfaction rate of the surgeon in the observation group was significantly higher than that in the control group (X2=19.64, P〈0.05). Conclusion The modified lateral position can effectively improve the stability of vital signs in the patients, reduce the incidence rate of postural complications, increase the comfort and safety, improve the satisfaction of the surgeon and meet the needs of quality care in the operation room.
