循证护理对减少腹腔镜手术后置管发生下肢静脉血栓及并发症的影响

Effect of evidence-based nursing on reducing venous thrombosis and complications of lower extremities induced by cathetering after laparoscopic surgery

目的探讨循证护理对减少腹腔镜手术后置管发生下肢静脉血栓及并发症的影响。方法选取我院2014年8月~2016年8月的100例腹腔镜手术后置管患者进行研究,将患者随机分为对照组和试验组各50例,其中对照组患者采用常规护理方式进行干预,试验组患者在对照组的基础上采用循证护理对患者进行护理干预,观察两组患者腹胀、感染、皮下气肿、下肢静脉血栓等发生情况;采用生活质量核心量表(QOL-30)对患者生活质量进行评估。结果两组患者护理干预后,试验组腹胀、感染、皮下气肿、下肢静脉血栓等发生率明显低于对照组,差异有统计学意义(P<0.05);护理后试验组的生活质量评分明显高于对照组,差异有统计学意义(P<0.05);护理后试验组满意度(98.0%)明显高于对照组(78.0%),差异有统计学意义(P<0.05)。结论对腹腔镜手术后置管患者进行循证护理干预能够明显的减少患者下肢静脉血栓及并发症发生,有效的改善患者生活质量,值得推广应用。

Objective To explore the effect of evidence-based nursing on reducing venous thrombosis and complications of the lower extremities induced by cathetering after laparoscopic surgery. Methods 100 patients receiving cathetering after laparoscopic surgery who were enrolled from August 2014 to August 2016 in our hospital were selected. The pa- tients were randomly divided into the control group and the experimental group, with 50 cases in each group. The pa- tients in the control group were treated with routine nursing methods, and the patients in the experimental group were treated with evidence-based nursing care on the basis of the control group. Bloating, infection, subcutaneous emphyse- ma, venous thrombosis of lower extremities and other complications were observed in both groups; patients quality of life was assessed using QOL-30. Results The incidence rate of bloating, infection, subcutaneous emphysema and ve nous thrombosis of lower extremities in the experimental group was significantly lower than that in the control group after nursing intervention, and the differences were statistically significant （P〈0.05）; the score of quality of life in the experi mental group after nursing was significantly higher than that in the control group, and the difference was statistically significant（P〈0.05）; the satisfaction rate in the experimental group（98.0%）was significantly higher than that in the con- trol group （78.0%）, the difference was statistically significant（P〈0.05）. Conclusion Evidence-based nursing intervention in the patients receiving cathetering after laparoscopic surgery can significantly reduce the venous thrombosis and com- plications of the lower extremities, and improve the quality of life of patients, which is worthy of popularization and ap- plication.
