慢性阻塞性肺疾病及睡眠呼吸障碍患者的免疫功能变化情况研究分析

Study analysis of changes of immune function in patients with chronic obstructive pulmonary disease and sleep disordered breathing

目的探讨慢性阻塞性肺疾病(COPD)及睡眠呼吸障碍(OSAHS)患者的免疫功能变化情况。方法选取2014年1月~2017年3月我院收治的46例COPD患者作为COPD组,39例OSAHS患者作为OSAHS组,40例重叠综合征(OS)患者作为OS组,比较三组的肺功能情况,采用流式细胞术检验三组的T淋巴细胞亚群CD4、CD8、CD4/CD8水平,分析组间的差异性。结果 COPD组的呼吸暂停低通气指数(AHI)显著低于OSAHS组和OS组,差异有统计学意义(P<0.05);OSAHS组的FEV1、FEV1/FVC水平显著高于COPD组和OS组,差异有统计学意义(P<0.05)。OS组的CD4、CD4/CD8水平显著低于COPD组和OSAHS组,差异有统计学意义(P<0.05);其他指标比较差异无统计学意义(P>0.05)。结论 OS患者的免疫功较COPD或OSAHS患者明显降低,呼吸机治疗能够改善患者的免疫功能,值得临床推广应用。

Objective To investigate the changes of immune function in patients with chronic obstructive pulmonary dis- ease （COPD） and sleep disordered breathing （OSAHS）.Methods From January 2014 to March 2017,46 patients with COPD treated in our hospital were selected as the COPD group,39 patients with OSAHS were selected as the OSAHS group,40 patients with overlap syndrome （OS） were selected as the OS group.The lung function of the three groups was compared,and the T lymphocyte subsets such as CD4,CD8 and CD4/CD8 levels in the three groups were examined by flow cytometry,and the difference between the three groups was analyzed.Results The apnea hypopnea index （AHI） of the COPD group was significantly lower than that of the OSAHS group and the OS group,with significant difference （P〈 0.05）.The levels of FEV1 and FEV1/FVC in OSAHS group was significantly higher than that in the COPD group and the OS group,with significant difference （P〈0.05）.The levels of CD4 and CD4/CD8 in the OS group was significantly lower than that in the OSAHS group and the COPD group,with significant difference （P〈0.05）.There was no significant difference in the other indicators between the three groups （P〉0.05）.Conclusion The immune function of patients with OS is significantly lower than that of patients with COPD or OSAHS.Ventilator therapy can improve the immune function of patients,and it is worthy of clinical promotion and application.
