高频超声对痛风性关节炎的早期诊断价值

Early diagnostic value of high frequency ultrasound in gouty arthritis

目的 探讨高频超声对痛风性关节炎（GA）的早期临床诊断价值,为临床的降尿酸治疗提供依据.方法 选择30例痛风性关节炎患者（组1）、30例无症状高尿酸血症患者（组2）和30例健康人作为对照组（组3）,对三组研究对象行双侧第一跖趾关节高频超声检查,收集和比较临床及血清学数据.结果 三组研究对象的第一跖趾关节炎性症状发生率有显著差异（P〈0.05）;三组研究对象的血尿酸水平有显著差异（P〈0.05）,三组研究对象的血肌酐水平无显著差异（P﹥0.05）;三组的高频超声检查的＆quot;双轨征＆quot;＆quot;暴风雪征＆quot;和＆quot;骨侵蚀＆quot;比较,差异均具有统计学意义（P〈0.05）.结论 高频超声是诊断亚临床痛风性关节炎的重要依据,具有无创性、方便可行、无辐射等优点,值得临床推广.

Objective To explore the value of high frequency ultrasound in early diagnosis of gouty arthritis. Methods Thirty cases of patients with gouty arthritis （group 1）, 30 cases of patients with asymptomatic hyperuricemia （group 2） and 30 cases of healthy people as control group （group 3） were selected as research objects. All objects＇ first metatarsophalangeal joint was examined with high frequency ultrasonography, the clinical and serological data were collected and analyzed. Re- suits There was significant difference among the three groups in the incidence of arthritis of the first metatarsophalangeal joint （P〈0.05）. The difference of serum uric acid among the three groups was significant （P〈0.05）, and there was no signifi- cant difference in serum creatinine among the three groups （P〉0.05）. Compared the high frequency ultrasound examination of the three groups according to the result of ＂double track sign snowstorm sign＂ and ＂bone erosion＂, the differences were statistically significant （P〈0.05）. Conclusion High frequency uhrasound is an important basis for the diagnosis of subclinical gouty arthritis, and it has the advantages of non-invasive, convenient and feasible, no radiation, which is worthy of promotion.
