脊柱减压治疗腰椎间盘突出症的疗效及肌电图分析

Efficacy and electromyogram analysis of spinal decompression in the treatment of lumbar disc herniation

目的观察脊柱减压治疗腰椎间盘突出症的效果并进行肌电图分析。方法选择2015年3月~2017年3月期间在我院骨科接受治疗的腰椎间盘突出症患者52例作为研究对象,随机分为观察组和对照组,每组26例,分别接受脊柱减压治疗和常规治疗,比较两组患者的ODI评分、VAS评分、直腿抬高试验结果和总有效率,并进行肌电图分析。结果观察组患者ODI评分(20.3±2.2)分,VAS评分(2.3±0.4)分,直腿抬高试验(72.5±2.4)°,总有效率为92.3%;对照组患者ODI评分(42.6±3.1)分,VAS评分(3.8±0.7)分,直腿抬高试验(60.5±1.9)°,总有效率为69.2%,组间差异有统计学意义(P<0.05)。观察组腓总神经NCV、腓总神经DL、胫神经NCV、胫神经DL四项指标均优于对照组,组间差异有统计学意义(P<0.05)。结论脊柱减压治疗腰椎间盘突出症疗效显著,患者功能恢复情况更好,生活自理能力更强,有较高的临床应用与推广价值。

Objective To observe the effect of spinal decompression on lumbar disc herniation and to analyze by electromyography. Methods 52 patients with lumbar disc herniation treated in our hospital from March 2015 to March 2017 were enrolled and randomly divided into observation group and control group, with 26 cases in each group. The observation group was treated with spinal decompression and the control group was given conventional treatment. The ODI score, VAS score, straight leg raising test results and the total effective rate were compared and the EMG analysis was performed between the two groups of patients. Results In the observation group,the ODI score was （20.3±2.2） points,（2.3±0.4）points of the VAS score,（72.5±2.4）° of the straight leg raising test, and the total effective rate was 92.3%. In the control group, the ODI score was （42.6±3.1）points, the VAS score was（3.8±0.7）points, the straight leg raising test was （60.5±1.9）°, and the total effective rate was 69.2% ,the differences were statistically significant between the two groups（P〈0.05）. The number of NCV, peroneal nerve DL, tibial nerve NCV and tibial nerve DL in the observation group were better than those in the control group, and the differences were statistically significant（P〈0.05）. Conclusion Spinal decompression is effective in the treatment of lumbar disc herniation. The patients have a better functional recovery, and stronger self-care ability. The spinal decompression has high clinical application and promotion value.
