经眶上外侧入路与单侧冠状入口经额入路治疗鞍区肿瘤对比观察

Comparative observation of lateral superaorbital approach and unilateral coronal entrance via frontal approach in the treatment of sellar tumors

目的探讨经眶上外侧入路与单侧冠状入口经额入路治疗鞍区肿瘤的效果。方法选择在我院治疗的50例鞍区肿瘤患者作为研究对象,根据入路方式的不同分为观察组(经眶上外侧入路)与对照组(单侧冠状入口经额入路),每组各25例。比较两组肿瘤切除情况、围手术期指标以及术后并发症发生情况。结果观察组肿瘤全切率为88.00%(22/25),对照组全切率为72.00%(18/25),两组肿瘤全切率相比,差异无统计学意义(P>0.05);观察组手术时间、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);对照组术后并发症发生率稍高于观察组,但差异无统计学意义(P>0.05)。结论鞍区肿瘤采用经眶上外侧入路与单侧冠状入口经额入路治疗均能取得较好效果,但经眶上外侧入路在缩短手术时间、减少出血量方面更具优势,有利于患者术后恢复,缩短住院时间。

Objective To investigate the effect of lateral superaorbital approach and unilateral coronal entrance via frontal approach on the treatment of sellar tumors. Methods 50 patients with sellar tumors treated in our hospital were selected as the subjects. According to the different approaches, the patients were divided into observation group （by lateral superaorbital approach） and the control group （unilateral coronal entrance by frontal approach）, with 25 patients in each group. The tumor resection, perioperative indicators and postoperative complications were compared between the two groups. Results The total tumor resection rate was 88.00% （22/25） in the observation group and 72.00% （18/25） in the control group. There was no significant difference between the two groups in the total tumor resection rate （P〉0.05）. The operation time and hospitalization time of the observation group were shorter than those of the control group, and the intraoperative blood loss of the observation group was less than that of the control group, with statistically signifi- cant difference （P〈0.05）. The incidence of postoperative complications in the control group was slightly higher than that in the observation group, but the difference was not statistically significant（P〈0.05）. Conclusion Sellar tumors can achieve very good results with lateral superaorbital approach and unilateral coronal entrance by frontal approach. How- ever, the lateral superaorbital approach has the advantage of shortening the operation time and reducing the blood loss, which is conducive to postoperative recovery of patients and shortening hospital stay.
