缬沙坦、苯磺酸氨氯地平片联用在老年高血压治疗中的效果观察

Effects of valsartan combined with amlodipine in the treatment of elderly hypertension

目的观察缬沙坦、苯磺酸氨氯地平片联用对老年高血压患者血管内皮功能的影响以及降压作用。方法选取2014年12月~2016年4月收治的老年高血压患者120例,随机分为联合组和对照组各60例,对照组给予苯磺酸氨氯地平片治疗,联合组在此基础上加用缬沙坦,治疗3个月,比较两组治疗前后内皮素-1、NO、收缩压、舒张压,参照Celer-majer方法测定静息状态下肱动脉内径。结果两组治疗后内皮素-1、NO水平均较治疗前明显改善,差异有统计学意义(P<0.05),且联合组改善较对照组更明显,差异均有统计学意义(P<0.05)。两组患者治疗后收缩压、舒张压水平与治疗前比较差异有统计学意义(P<0.05),并且联合组较对照组血压下降更明显,差异有统计学意义(P<0.05)。两组治疗后肱动脉内径变化百分数较治疗前明显升高(P<0.05),并且联合组较对照组升高更明显(P<0.05)。结论缬沙坦、苯磺酸氨氯地平片联用对老年高血压患者血管内皮功能有明显改善作用,高血压治疗效果显著,安全可靠。

Objective To observe the effect of valsartan combined with amlodipine besylate tablets on vascular en- dothelial function in elderly hypertensive patients and its antihypertensive effect. Methods 120 cases of elderly hyper- tensive patients admitted from December 2014 to April 2016 were randomly divided into two groups, including the combined group（n=60） and the control group（n=60）. The control group was treated with amlodipine besylate tablets. The combined group was given valsartan based on the treatment of the control group for 3 months. The levels of endothelin-1, NO, systolic blood pressure and diastolic blood pressure were compared before and after treatment between the two groups. The brachial artery diameter was measured referring to the Celer-majer method. Results The levels of endothe- 1in-1 and NO in the two groups were significantly improved compared with those before treatment（P〈O.05）, and the improvement of the combined group was more obvious than that of the control group （P〈0.05）. There were significant dif ferences in systolic blood pressure and diastolic blood pressure in the two groups between before and after surgery （P〈 0.05）. And the blood pressure of the combined group was significantly lower than that of the control group, and the dif- ference between the two groups was significant （P〈0.01）. The percentage of brachial artery internal diameter change in the two groups was significantly higher than that before treatment（P〈O.05）, and the improvement of the combined group was more obvious than that of the control group （P〈0.05）. Conclusion Valsartan combined with amlodipine besylate tablets has a significant effect on vascular endothelial function in elderly hypertensive patients. And its antihypertensive treatment is effective and safe.
