基于数据挖掘的刘健教授治疗强直性脊柱炎用药规律研究

Study on Professor LIU Jian's Medication Rules for Ankylosing Spondylitis Based on Data Mining

目的:应用数据挖掘技术和关联规则分析刘健教授治疗强直性脊柱炎中药配伍及组方规律。方法:基于数据挖掘技术,对2016年3月至2017年3月刘健教授治疗强直性脊柱炎门诊患者病例进行回顾性研究,采用SPSS 22.0与SPSS Modeler 14.2进行药物及其性味归经频数分析、聚类分析及关联规则。结果:(1)药物性味归经前三位:寒性、温性和平性,甘味、苦味和辛味以及归肝、脾、胃经药物使用最频繁。(2)刘健教授常用药物一般分为4类:健脾祛湿药、清热药、活血化瘀药、祛风湿药,多数药物使用在50%以上。(3)关联规则分析提取出健脾化湿药茯苓及活血化瘀药红花与清热药、活血化瘀药、祛风湿药、补益药是相互关联的。(4)聚类分析得出桃仁、红花、鸡血藤、路路通、威灵仙,黄芩、白花蛇舌草、蒲公英、杜仲、丹参、厚朴、豨莶草,姜半夏、山药,炒谷芽、炒麦芽、陈皮,黄芪、当归、茯苓,薏苡仁等6组。结论:刘健教授治疗强直性脊柱炎以从脾论治为主,配伍清热解毒、祛风除湿、活血化瘀中药,寒温并用,甘苦同施,肝脾肾同调,祛邪不忘扶正,标本兼治。

Objective：To analyze the compatibility and prescription rules of Professor Liu Jian in the treatment of ankylosing spondylitis based on Data Mining and Association Rule.Methods：Based on Data Mining,a retrospective study was made for patients with ankylosing spondylitis treated by Professor LIU Jian from March 2016 to March 2017,using SPSS 22.0 and SPSS Modeler 14.2 to analyze the channel entry frequency of medicinals and their natures and flavors,to make a cluster analysis and to analyze the association rules.Results： （1） According to the channel entry frequency, the top three natures are cold,warm and neutral.Sweet,bitter and pungent medicinals acting on the liver, spleen and stomach channels are most frequently used. （2） The medicinals commonly used by Professor LIU Jian were generally divided into four categories：spleen-invigorating and dampness-removing medicinals,heat-clearing medicinals,blood-activating and stasis-resolving medicinals,and wind-damp-dispelling medicinals,most of which were used more than 50%. （3） According to the analysis of association rules,fú ling （ a spleen-invigorating and dampness-removing medicinal ） and hóng huā （ a blood-activating and stasis-resolving medicinal ） were interrelated with heat-clearing medicinals,blood-activating and stasis-resolving medicinals,wind-damp-dispelling medicinals and tonifying medicinals. （4） By cluster analysis,there were six groups of medicinals：táio rán,hóng huā,jī xuè tǒng,lù lù tǒng and wēi ling xiān;huámg qín,bái huā shé shé cāo,pú gōng yīng,dù zhòng,dān shēn,hòu pò and xī xiān cǎo;jiǎng bàn xià and shān yào;fried gǔ yá and chén pi;huáng qi,dāng guī and fú ling;yì yǐ rén.Conclusion：For ankylosing spondylitis,Professor LIU Jian diagnoses and treats it from spleen,using heat-clearing and toxin-resolving medicinals,wind-damp-dispelling medicinals and blood-activating and stasis-resolving medicinals,which are both cold and warm,sweet and bitter, regulating the liver and the spleen,dispelling pathogen and reinforcing healthy qi,and treating the root and branch simultaneously.
