补肾抗风湿方联合西药治疗类风湿关节炎继发骨质疏松的疗效观察

Clinical effect of the Bushen Kangfengshi Recipe combined with western medicine in the treatment of the secondary osteoporosis in patients with rheumatoid arthritis

目的:探讨补肾抗风湿方联合西药治疗类风湿关节炎继发骨质疏松的临床疗效。方法:类风湿关节炎继发骨质疏松病人122例,随机分为观察组和对照组,各61例。对照组予常规西药甲氨蝶呤、来氟米特进行治疗,观察组在对照组基础上予补肾抗风湿方,2组均治疗6个月。比较2组病人治疗前后晨僵时间、关节肿胀指数、关节压痛指数、关节功能指数和血钙、血小板计数(PLT)、红细胞沉降率(ESR)、类风湿因子(RF)、C反应蛋白(CRP)水平及腰椎、股骨颈、Ward三角、大转子骨密度,并于治疗后评价2组的临床疗效。结果:与治疗前比较,治疗后2组晨僵时间、关节压痛指数、关节肿胀指数、关节功能均显著改善(P<0.01),且观察组改善情况均显著优于对照组(P<0.01)。与治疗前比较,治疗后2组PLT、ESR、RF、CRP均显著改善(P<0.01),观察组血钙亦较治疗前显著改善(P<0.01),且观察组治疗后PLT、ESR、RF、CRP、血钙改善情况均显著优于对照组(P<0.01)。观察组总有效率为91.8%,高于对照组的75.4%(P<0.05)。观察组治疗后腰椎、股骨颈、Wald三角、大转子部位骨密度均较治疗前和对照组治疗后显著改善(P<0.01)。结论:补肾抗风湿方联合西药治疗类风湿关节炎继发骨质疏松的临床疗效好,可显著改善病人临床症状和骨代谢情况。

Objective：To observe the curative effects of the Bushen Kangfengshi Recipe combined with western medicine in the treatment of the secondary osteoporosis in patients with rheumatoid arthritis.Methods：One hundred and twenty-two rheumatoid arthritis patients with secondary osteoporosis were randomly divided into the observation group and control group（61 cases each group）.The control group was treated with methotrexate and leflunomide,and the observation group was additionally treated with Bushen Kangfengshi Recipe based on the control group for 6 months.The morning stiffness time,index of joint swelling,joint tenderness and joint function,blood calcium level,platelet（PLT） count,erythrocyte sedimentation rate（ESR）,rheumatoid factor（RF）,c-reactive protein（CRP） level,lumbar spine,femoral neck,Ward triangle and greater trochanter bone density between two groups were compared.The clinical effects in two groups were evaluated after treatment.Results：Compared with before treatment,the morning stiffness time,joint tenderness index,joint swelling index and joint function in two groups were obviously improved after treatment（P〈0.01）,and the improvement degree of which in observation group was significantly better than that in control group（P〈0.01）.Compared with before treatment,the PLT,ESR,RF and CRP in two groups were obviously improved after treatment（P〈0.01）.The blood calcium level in observation group was significantly improved after treatment（P〈0.01）,and the improvement degree of which in observation group was significantly better than that in control group（P〈0.01）.The total effective rate in observation group（91.8%） was significantly higher than that in control group（75.4%）（P〈0.05）.Compared with before treatment and control group,the lumbar spine,femoral neck,Ward triangle and greater trochanter bone density in observation group were significantly improved after treatment（P〈0.01）.Conclusions：The curative effects of the Bushen Kangfengshi Recipe combined with western medicine in the treatment of the secondary osteoporosis in patients with rheumatoid arthritis are good,which can significantly improve the clinical symptoms and bone metabolism of patients.
