心包腔内置管引流并注入顺铂对恶性心包积液的治疗效果

目的:观察心包腔内置管引流并注入顺铂治疗恶性心包积液的临床疗效。方法:回顾性分析对23例恶性心包积液的患者,采用中心静脉导管经皮心包穿刺留置引流心包积液,待充分引流后注入顺铂。结果:所有患者一次穿刺置管引流成功,无严重并发症,完全缓解7例,部分缓解13例,无效3例,总有效率86.95%。23例患者中位生存期为153 d(35～731 d)。结论:心包腔内置管引流并注入顺铂治疗恶性心包积液安全有效,能够改善预后,延长生存,值得临床推广应用。

Objective To observe the pericardial cavity drainage and injection of clinical curative effect of cisplatin in the treatment of malignant pericardial effusion.Methods A retrospective analysis of 23 cases with malignant hydropericardium in patients with,the use of percutaneous pericardial puncture and indwelling central venous catheter drainage for pericardial effusion,be sufficient drainage after injection of cisplatin.Results All patients with a puncture catheter drainage success,no serious complications,complete remission in 7 cases,some 13 cases,is not valid in 3 cases,total effective rate 86.9%.Lifetime 153D in 23 patients(35-731d).Conclusion The pericardial cavity drainage and injection and cisplatin in the treatment of malignant pericardial effusion,safe and effective,can improve the prognosis,extended life,and clinical application.
