低分子肝素皮下注射治疗重症急性胰腺炎临床观察

Clinical Observation of Low Molecular Heparin by Subcutaneous Injections in Treatment of Severe Acute Pancreatitis

目的探讨低分子肝素(low molecular weight heparin,LMWH)治疗重症急性胰腺炎(severe acute pancreatitis,SAP)的临床效果及安全性。方法选择2014年3月—2016年1月收治的SAP 120例,按治疗方法分为观察组和对照组,每组60例。对照组给予常规治疗,观察组在对照组基础上给予LMWH皮下注射。观察治疗2周后中转手术率,比较2组临床症状恢复正常及总住院时间、近期疗效,记录并发症发生情况。结果观察组肠鸣音、体温恢复正常、腹痛消失及总住院时间均短于对照组(P<0.05);观察组临床总有效率高于对照组,中转手术率和并发症发生率低于对照组(P<0.05)。结论 LMWH治疗SAP临床疗效较好,可降低中转手术率和并发症发生率,安全性较高。

Objective To investigate clinical effect and safety of low molecular weight heparin （LMWH） in treatment of severe acute pancreatitis （SAP）. Methods A total of 120 SAP patients admitted during March 2014 and January 2016 were divided into observation group （n = 60） and control group （n = 60） according to treatment methods. Control group was treated with conventional treatment, while observation group was added with LMWH by subcutaneous injection on the basis of treatment for control group. In two groups, transference operation rates after treatment for two weeks were observed ; recovery time of clinical symptoms, total hospitalized time and short-time effect were compared ; in- cidence rates of complications were recorded. Results Recovery times of clinical symptoms such as bowel sound, body temperature and abdominal pain, and total hospitalized time were all shorter than those in control group （P 〈 0.05 ）. In observation group, the total effective rate was significantIy higher, while transference operation rate and incidence rate of complications were significantly lower （P 〈 0. 05）. Conclusion LMWH in treatment of SAP can achieve good clinical effect, and decrease transference operation rate and incidence rate of complication with good safety.
