个案管理模式在乙型肝炎母婴阻断中的应用

Application of case management model in blocking of mother-to-child transmission of hepatitis B

目的:探讨个案管理模式在病毒性乙型肝炎母婴阻断中的应用效果。方法:选取2015年1~12月我院产科门诊建档并住院分娩的HBs Ag阳性孕妇156例,按门诊产检建档时间先后顺序分为观察组74例和对照组72例。对照组采用常规管理模式,观察组采用个案管理模式。采用自行设计的调查问卷测试产妇对HBV母婴阻断知识掌握情况,在婴儿出生7,12个月时进行HBV DNA、HBs Ag、HBs Ab、HBe Ag、HBe Ab、HBc Ab定量检测。结果:观察组产妇HBV母婴阻断知识掌握合格率高于对照组(P<0.05),观察组7月龄婴儿HBs Ab阳性率高于对照组(P<0.05)。结论:个案管理模式有助于提高HBV母婴阻断治疗依从性。

Objective：To discuss the application effect of case management model in blocking of mother-to-child transmission of viral hepatitis B.Methods：Selected 156 pregnant women with positive HBsAg filed, hospitalized and delivering in obstetrical outpatient department of our hospital from January to December 2015, and they divided into 74 cases in the observation group and 72 cases in the control group according to the sequence of filing of outpatient obstetric examination, where conventional management model was employed for the pregnant women in the control group and case management model was employed for the pregnant women in the observation group.Self-made questionnaire was used to test the lying-in women＆#39;s mastering of HBV mother-to-child transmission blocking knowledge, and the lying-in women received HBV DNA,HBsAg,HBsAb,HBeAg,HBeAb and HBcAb quantitative examination 7 and 12 months after childbirth.Results：The conformance rate of HBV mother-to-child transmission blocking knowledge mastering of lying-in women in the observation group was higher than that of lying-in women in the control group （P〈0.05）, and the positive rate of BHsAb of 7 months＆#39; babies of lying-in women in the observation group was higher than that of lying-in women in the control group （P〈0.05）.Conclusion：Case management model could improve the treatment compliance of HBV mother-to-child transmission blocking.
