银川市农村居民家庭灾难性卫生支出现状及影响因素研究

A study on the status and influencing factors of catastrophic health expenditure of rural households in Yinchuan

目的了解银川市农村居民家庭灾难性卫生支出的现状,探索灾难性卫生支出的主要影响因素。方法采用分层随机抽样法抽取银川市400户农村家庭进行入户调查,共调查1 036人,运用卡方检验分析农村居民灾难性卫生支出的情况,采用二分类logistic回归分析影响家庭发生灾难性卫生支出的主要因素。结果以40%为界定标准,银川市农村居民家庭灾难性卫生支出的发生率为31.09%,平均差距为17.43%,相对差距为39.52%。经过二分类logistic回归分析得出,影响家庭灾难性卫生支出的主要因素有家庭人均年收入、户主受教育程度、家庭中是否有65岁及以上老年人、家庭中是否有慢性病人以及家中是否有人住院。结论家庭人均年收入较低(≤10 000元)的人群灾难性卫生支出率发生较高,家中有慢性病患者、住院病人及65以上老人较易发生灾难性卫生支出。表明应进一步完善医疗保障制度,并关注老年人口和慢性病的防治,提高教育水平,减少农村家庭灾难性卫生支出的发生。

OBJECTIVE To understand the current situation of catastrophic health expenditure of rural households in Yinchuan, and to explore the main factors affecting the catastrophic health expenditure. METHODS Using stratified random sampling in Yinchuan city in 400 rural households survey, a total of 1036 people were investigated, using the analysis of residents of catastrophic health expendi- ture in rural areas of the chi square test, logistic regression was used to analyze the main influencing factors of family catastrophic health payment. RESULTS 40% was defined as the standard of rural areas in Yinchuan.The incidence of catastrophic health expendi- ture was 31.09%, with an average difference of 17.43% and a relative difference of 39.52%. After binary logistic regression, the main factors affecting family catastrophic health expenditure are per capita annual income of the family, the education level, whether The family has the old man 65 years of age and older, whether the family has any chronic patients and in hospital. CONCLUSION the rate of catastrophic health expenditure is higher in the family which has a low annual income. And the family which has patients with chronic disease in hospital or has the elderly more than 65 is more likely subject to catastrophic health expenditure. It is suggested that the health care system should be further improved, and the prevention and treatment of the elderly population and chronic diseas- es should be paid attention to.
