摘要
目的:探讨微柱凝胶技术在ABO新生儿溶血病患儿输血前检验中的应用效果。方法:回顾性分析选取的40例ABO-HDN患儿的临床资料,随机取10份ABO同型红细胞制剂的血样,分别采用凝聚胺技术以及微柱凝胶技术开展主侧交叉配血,对两种配血方法的效果进行评估,同时对两种配血方法同直抗试验的相关性进行评估。结果:交叉配血共200次,微柱凝胶技术血清、放散液分别出现不和148、124次,凝聚胺技术血清、放散液分别出现不和6、10次。除凝聚胺交叉配血外,直抗阳性的血清和放散液交叉配血不合率高于直抗阴性(P<0.05)。两种检测技术在患儿血清和放散液交叉配血结果中比较,差异均无统计学意义(Kappa=0.019,P=0.300;Kappa=0.058,P=0.083),微柱凝胶技术灵敏度更高。结论:对新生儿开展输血前检查工作时,采用直接抗人球试验进行检测十分重要,且微柱凝胶技术在ABO新生儿溶血病患儿输血前检验中的应用效果显著,值得在临床上推广应用。
Objective: To investigate the application effect of microcolumn gel technique in pre-transfusion test of neonatal hemolytic disease in children with ABO.Method: 40 patients with ABO-HDN were selected, the clinical data of the selected subjects were analyzed retrospectively, 10 samples of ABO erythrocyte preparation wererandomly selected, using polybrene technique and microcolumn gel technology to carry out the main side of cross matching, the effects of the two methods were evaluated, at the same time, two kinds of matching methods with direct correlation between resistance test were evaluated.Result : Cross matching of blood 200 times, the serum and elution liquid appeared 148 times and 124 times respectively with microcolumn gel technique, polybrene technique in serum and elution liquid respectively with 6, 10 times.In addition to the crossed blood of the coagulation amine, direct antibody positive serum and elution liquid crossmatching incompatibility were higher than those of direct negative resistance ( P〈0.05 ) .Comparison of two kinds of detection in serum and elution crossmatch results, there were no significant differences ( Kappa=0.019, P=0.300; Kappa=0.058, P=0.083 ) .Microcolumn gel technique in higher sensitivity.Conclusion: To carry out inspection work on neonatal before transfusion, using direct antiglobulin test detection is very important, and the effect of microcolumn gel technique in the pre-transfusion test of neonatal hemolytic children with ABO is significant, it is worthy of clinical application.
