摘要
目的:探讨Syngo TWIST超快速四维血管成像引导下动脉溶栓治疗急性脑梗死的临床疗效及其对患者神经功能的影响。方法:选取2014年1月-2015年6月本院收治的68例急性脑梗死患者为研究对象,随机分为对照组和研究组,每组34例。对照组给予阿替普酶行静脉溶栓治疗,而研究组则在Syngo TWIST超快速四维血管成像的引导下给予阿替普酶行动脉溶栓治疗。于治疗前和治疗后2 h、12 h、1 d、7 d、14 d和30 d采用美国国立卫生研究院卒中量表(NIHSS)分别对患者进行神经功能缺损评分,比较两组患者的临床疗效、并发症发生情况和预后情况,治疗前和治疗后14 d患者血清TNF-α和IL-8的水平。结果:研究组患者治疗后各个时段的NIHSS评分均显著低于对照组,差异均有统计学意义(P<0.05),研究组患者治疗后14 d血清TNF-α和IL-8水平均低于对照组,差异均有统计学意义(P<0.05),研究组治疗总有效率及预后良好率均优于对照组,差异均有统计学意义(P<0.05),研究组并发症发生率低于对照组,差异有统计学意义(P<0.05),死亡率低于对照组,但差异无统计学意义(P>0.05)。结论:Syngo TWIST超快速四维血管成像引导下动脉溶栓治疗急性脑梗死具有良好的临床疗效和安全性,同时可降低患者血清TNF-α和IL-8水平,改善患者的神经功能和预后,值得应用于临床。
Objective: To discuss the effect of nerve function in patients with acute cerebral infarction treated with arterial thrombolysis under the guidance of Syngo TWIST technology.Method: From January 2014 to June 2015, 68 acute cerebral infarction patients treated in our hospital were selected, and divided into research group and control group according to the random number table method, 34 patients in each group.Contr01 group was treated with intravenous thrombolysis used Aheplase, research group was treated with arterial thrombolysis under the guidance of Syngo TWIST technology, Graded nerve function defect by NIHSS before treatments and after treatments 2 h, 12 h, 1 d, 7 d, 14 d and 30 d of two groups, the clinical effect, eomplications and prognosis condition, serum levels of TNF-a and IL-8 before and after treatment 14 d of two groups were compared. Result: The NIHSS in different times of research group after treatments were lower than those of control group, the differences were statistically significant ( P〈0.05 ) ; serum TNF- aand IL-8 levels of research group were lower than those of control group, the differences were statistically significant ( P〈0.05 ) ; total treatment effective rate and favourable prognosis rate of research group were better than those of control group, the differences were statistically significant ( P〈0.05 ) ; complication occurrence rate of research group were lower than those of control group, the different was statistically significant ( P〈0.05 ) ; mortality rate of research group were lower than those of control group, but there was no significant difference (P〉0.05) .Conclusion: Arterial thrombolysis under the guidance of Syngo TWIST technology for acute cerebral infarction patients, has efficacy and safety, can decrease the serum TNF-oL and IL-8 levels, improve the nerve function and prognosis, which is worthy of clinical am^lication.
