血清H-FABP和cTnT水平联合测定在急性心肌梗死诊断中的作用 被引量:18

The role of combined detection of serum H-FABP and cTnT levels in the diagnosis of acute myocardial infarction

摘要 目的:研究心肌损伤标志物心型脂肪酸结合蛋白(H-FABP)和心肌肌钙蛋白T(cTnT)联合检测在急性心肌梗死(AMI)临床诊断中的作用和意义。方法:采用ELISA方法对33例AMI患者(AMI组)和31例正常对照者(对照组)血清中的H-FABP和cTnT浓度进行测定,比较两组血清样本中H-FABP与cTnT的浓度,并考察HFABP、cTnT及其组合的诊断灵敏度、特异度以及诊断准确率。结果:与对照组相比,AMI组患者血清H-FABP和cTnT浓度均显著增高(P<0.05)。H-FABP、cTnT、H-FAB+cTnT的灵敏度分别为60.61%、75.76%、96.97%,特异度分别为:83.55%、85.77%、93.55%,诊断准确率分别为85.94%、85.94%、95.31%。结论:血清H-FABP和cTnT浓度联合测定可为临床AMI患者的确诊提供准确的实验室依据,有助于减少漏诊及误诊。 Objective:To study the clinical significance of myocardial injury markers cardiac fatty acid binding protein(H-FABP)and cardiac troponin T(cTnT)in clinical diagnosis of acute myocardial infarction(AMI).Method:The serum H-FABP and cTnT levels in 33 patients with AMI(AMI group)and 31 normal controls(control group)were determined by double sandwich enzyme-linked immunosorbent assay(ELISA).Then levels of HFABP and cTnT in the two groups were compared,and the sensitivity,specificity,diagnose accordance rate of the indexes were investigated.Result:Compared with normal control group,AMI group had significantly higher serum H-FABP and cTnT concentrations(P0.05).The sensitivity of H-FABP,cTnT and H-FAB plus cTnT were60.61%,75.76% and 96.97% respectively.The specificity were 83.55%,85.77% and 93.55% respectively.The diagnose accordance rate were 85.94%,85.94% and 95.31% respectively.Conclusion:The combined determination of serum H-FABP and cTnT can provide accurate laboratory basis for the diagnosis of AMI,which is helpful to reduce misdiagnosis and misdiagnosis.
