自发性冠状动脉夹层的CT诊断

Diagnosis of spontaneous coronary artery dissection by multidetector computed tomography

目的通过对6例自发性冠状动脉夹层的分析,揭示多层螺旋CT对冠状动脉夹层的评估价值。方法回顾性分析我院6例自发性冠状动脉夹层患者的临床资料、CTCA、CAG、血管内超声(Intravascular ultrasound,IVUS)影像和术后随访资料等。结果 6例自发性冠状动脉夹层患者中,CTCA表现为局限性管腔狭窄伴新月形血栓围绕2例,可见内膜撕裂片5例。结论对于自发性冠状动脉夹层的诊断,多层螺旋CT冠状动脉成像是一种无创、敏感、精确的检查方法。

Objective Through the analysis of 6 cases of spontaneous coronary artery dissection, to reveal the diagnostic value of MDCT in detecting coronary artery dissection.Methods We retrospectively analyzed the clinical data images of CTCA, CAG, IVUS and follow-up data of 6 patients with spontaneous cornary dissection in our hospital.Results In 6 cases with spontaneous coronary artery dissection, CT features included narrowed lumen surrounded by crescent-shaped hematoma in 2 cases and visible intimal flap in 5 cases.Conclusion Multidetector CT coronary artery angiography is a sensitive and accurate technique to diagnose spontaneous coronary artery dissection.
