高胰岛素血症与结直肠癌发生、发展的相关性研究

Study on the relationship between hyperinsulinemia and colorectal cancer occurrence and development

目的研究并探讨高胰岛素血症与结直肠癌发生、发展的相关性,为临床防治结直肠癌提供科学依据。方法选取2014年1月—2016年12月期间我院收治的60例结直肠癌患者作为观察组,另随机选取同期在我院参加健康体检的100例体检正常者作为对照组,比较其空腹胰岛素、胰岛素抵抗指数、游离脂肪酸、空腹血糖,并根据结直肠癌患者的空腹胰岛素水平将其分为高胰岛素血症组、胰岛素正常组,比较其TNM分期、淋巴结转移、远处转移情况。结果相比于对照组,观察组的空腹胰岛素、胰岛素抵抗指数、游离脂肪酸、空腹血糖均明显更高(P<0.05);高胰岛素血症组患者的TNM Ⅲ~Ⅳ期例数、淋巴结转移例数、远处转移例数均明显多于胰岛素正常组(P<0.05)。结论高胰岛素血症与结直肠癌的发生、发展密切相关,结直肠癌患者的胰岛素抵抗水平可在一定程度上反映其病情进展。

Objective： To study the relationship between hyperinsulinemia and the occurrence and development of color- eetal cancer, and to provide scientific basis for the prevention and treatment of eoloreetal cancer. Methods：Sixty eases of eolorectal cancer patients in our hospital during the period from January 2014 to December 2016 as the observation group, and 100 healthy subjects were selected to participate in the same period of health examination in our hospital as the control group. The fasting insulin, insulin resistance index, free fatty acid and fasting blood glucose were compared, and the pa- tients with colorectal cancer were divided into high insulin and normal insulin group according to the fasting insulin levels, and the TNM staging, lymph node metastasis and distant metastasis were compared. Results： Compared with control group, fasting insulin, insulin resistance index, free fatty acid, fasting blood glucose levels were significantly higher in the obser- vation group （P 〈 0.05 ） ;the number of cases of TNM Ⅲ-Ⅳ, h lymph node metastasis cases, and distant metastasis ea- ses in the patients with hyperinsulinemia were significantly higher than those in normal insulin group （P 〈 0.05） ; occur- rence and development of hyperinsulinemia and colorectal cancer were closely related. Conclusion： hyperinsulinemia is closely related to the occurrence and development of eoloreetal cancer, and the level of insulin resistance in patients with eoloreetal cancer may reflect the progression of the disease to a certain extent.
