同种异体骨与人工骨治疗Sanders Ⅲ型跟骨骨折临床研究

Clinical Study of Allogeneic Bone and Artificial Bone in the Treatment of Sanders Type Ⅲ Calcaneal Fracture

目的探讨跟骨骨折Sanders分型中Ⅲ型骨折切开复位植骨内固定选取同种异体骨与人工骨植骨术后疗效,从而为跟骨骨折的植骨治疗提供临床治疗依据。方法分析2014年9月~2016年9月因跟骨SandersⅢ型骨折入我科行跟骨骨折切开复位植骨内固定术45例患者的病例资料,观察分析术后刀口愈合、骨折复位情况。结果同种异体骨植入组创面愈合时间长于人工骨植入组(P<0.05),术后X片分析Bohler角、Gissane角等差异无统计学意义(P>0.05)。结论同种异体骨与人工骨在跟骨骨折治疗中骨折复位情况无明显差异,但同种异体骨排异反应更明显,导致创面渗出、愈合时间长。认为选用人工骨植骨治疗跟骨骨折优于同种异体骨。

Objective To discuss the Sanders classification of calcaneal fractures Ⅲ type fractures open reduction and internal fixation of bone graft selection of allogeneic bone and artificial bone graft surgery treatment curative effect,so as to provide the clinical treatment basis for bone grafting treatment of calcaneal fractures.Methods Analysis of September 2014 ～ September 2016 due to calcaneal Sanders type Ⅲ fractures into my department calcaneal fracture incision reduction of bone graft in 45 cases of patients with data to observe the analysis of postoperative wound healing,fracture reduction.Results Allogeneic bone implant group wound healing time is longer than the artificial bone implant group,postoperative X-ray visible bohler angle,angle of gissane no statistical difference.Conclusion All ogeneic bone and artificial bone fracture in the treatment of calcaneal fractures have no obvious difference,but the allograft rejection is more apparent,cause wounds effusion,healing time is long,because of the open reduction with bone skin characteristic of the incision,the author thinks that choose artificial bone graft in the treatment of calcaneal fractures is better than that of allograft bone.
