基于Barthel指数评定量表的临床分层管理在结核病房中的应用效果

Application effects of clinical stratification management based on Barthel index rating scale in tuberculosis ward

收集2014年11月至2016年10月青岛市胸科医院胸内科五病房收治的自理能力重度和中度依赖患者,将2014年11月至2015年10月的65例患者作为对照组,实施常规性护理;将2015年11月至2016年10月的60例患者作为观察组,实施基于Barthel指数评定分值的分层护理管理.观察组并发症、护患纠纷事件、护理风险事件的发生率[3.33％ （2/60）、5.00％ （3/60）、6.67％（4/60）]均低于对照组[13.85％（9/65）、18.46％（12/65）、20.00％（13/65）],差异均有统计学意义（χ^2=4.30、5.35、4.72,P值均＜0.05）.观察组患者或家属对护理质量满意度的评分中,健康教育、心理护理、服务态度、基础护理、操作性护理的分值[（3.85±0.45）分、（3.79±0.53）分、（3.65±0.78）分、（3.38±0.56）分、（3.49±0.29）分]均高于对照组[（1.23±0.23）分、（1.14±0.22）分、（1.05±0.36）分、（1.16±0.42）分、（1.34±0.26）分],差异均有统计学意义（t=40.82、36.36、23.80、24.97、43.47,P值均＜0.01）.故认为基于Barthel指数评定量表在结核病房临床护理中实施分层管理能合理有效地配置病房人力资源,提高护理质量,降低患者护理风险事件的发生,提高患者家属满意度.

A total of 60 patients whose self-care abilities were severe or moderate dependent and stayed at the Thoracic Medicine Department of Qingdao Chest Hospital from November 2014 to October 2016 were involved in the study as the observation group,and the stratified nursing management based on the Barthel index score were provided to those patients.Sixty-five patients who stayed at the same ward in Qingdao Chest Hospital from November 2014 to October 2015 were selected as the control group and the routine nursing services were provided to those patients.In the observation group,the incidence of complications （3.33%,2/60）,the rate of the disputes between nurses and patients （5.00%,3/60）,the incidence of nursing risk events （6.67%,4/60）,were lower than those in the control group （13.85% （9/65）,18.46% （12/65）,20.00% （13/65））,and the differences were statistically significant （χ^2 =4.30,5.35,4.72,P〈0.05）.In the the observation group,the satisfaction scores of the patients or their family members to the nursing quality were as follows：health education （3.85±0.45）,psychological nursing （3.79±0.53）,service attitude （3.65±0.78）,basic nursing （3.38±0.56）,operation nursing,（3.49±0.29）,and they were all higher than the scores in the control group （（1.23 ± 0.23）,（1.14 ± 0.22）,（1.05 ± 0.36）,（1.16±0.42）,（1.34 ± 0.26））,the differences were statistically significant （t =40.82,36.36,23.80,24.97,43.47,P〈0.01）.It is concluded that by implementing the stratified management based on the Barthel index rating scale in the clinical nursing practice in the tuberculosis ward,the human resources can be more rationally and effec tively allocated in the ward,the quality of nursing services can be improved,the nursing risk events happened in the patients can be reduced,and the satisfaction degree of the patients or their family members can be improved.
