2型糖尿病脂蛋白相关磷脂酶A2水平与颈动脉内膜中层厚度的关系 被引量:4

Relationship between Lipoprotein-Associated Phospholipase A2 Levels and Carotid Intima-Media Thickness in Type 2 Diabetes Mellitus

摘要 目的观察2型糖尿病患者血清脂蛋白相关磷脂酶A2(LP-PLA2)表达情况,探讨其与颈动脉内膜中层厚度(IMT)的相关性。方法选择100例2型糖尿病患者为观察组,100例健康体检者为对照组,检测2组受试者血清LP-PLA2水平、IMT及其他相关生化指标。根据IMT有无增厚将观察组分为A组(IMT≤1.0 mm)与B组(IMT>1.0mm),以IMT值为因变量,行单因素Logistic回归分析LP-PLA2与IMT的相关性。结果观察组血清LP-PLA2、餐后2h血糖(2hPBG)、糖化血红蛋白(HbA1c)、空腹血糖(FPG)、甘油三酯(TG)水平及IMT值均较对照组明显升高(P<0.05)。A组46例患者与B组54例患者血清FPG、2hPBG、TG、HbA1c水平比较差异无统计学意义(P>0.05);B组血清LP-PLA2水平及IMT值显著高于A组(P<0.05),经Logistic回归分析显示,血清LP-PLA2表达升高,是2型糖尿病患者IMT增厚独立相关因素(Wald=12.674,OR=10.512,P<0.05)。结论 2型糖尿病患者血清LP-PLA2水平较高,且LP-PLA2表达与IMT具有相关性,对该指标进行检测可为临床诊治提供有效参考。 Objective To observe the expression of serum lipoprotein-associated phospholipase A2(LP-PLA2),and to investigate its relationship to carotid intima-media thickness(IMT)in patients with type 2diabetes mellitus(T2DM).Methods Serum LP-PLA2 levels,IMT and related biochemical indexes were measured in 100 T2DM patients(observation group)and 100 healthy subjects(control group).Furthermore,these T2DM patients were divided into two groups:group A(IMT≤1.0mm)and group B(IMT〉1.0mm).The relationship between LP-PLA2 and IMT was analyzed using univariate Logistic regression analysis with IMT as the dependent variable.Results Compared with control group,serum levels of LP-PLA2,2-hour postprandial blood glucose(2hPBG),glycated hemoglobin(HbA1c),fasting plasma glucose(FPG),triglycerides(TG)and IMT significantly increased in observation group(P〈0.05).There were no significant differences in serum FPG,2hPBG,TG and HbA1 clevels between group A(n=46)and group B(n=54)(P〉0.05).However,serum LP-PLA2 levels and IMT in group B were significantly higher than those in group A(P〈0.05).The Logistic regression analysis showed that the increased expression of serum LP-PLA2 was an independent factor associated with IMT thickening in patients with T2DM(Wald=12.674,OR=10.512,P〈0.05).Conclusion Serum LP-PLA2 increases in patients with T2DM,and its expression is correlated with IMT.Therefore,the detection of LPPLA2 can provide effective reference for clinical diagnosis and treatment.
