非骨水泥型假体全髋关节置换术对老年股骨颈骨折的疗效

Curative Efficacy of Non-Cemented Total Hip Arthroplasty in Femoral Neck Fractures in Elderly Patients

目的探讨非骨水泥型假体全髋关节置换术对老年股骨颈骨折的临床疗效。方法将120例股骨颈骨折患者按手术方式的不同分为2组:观察组和对照组,每组60例。观察组患者采用非骨水泥型假体全髋关节置换术,对照组患者采用骨水泥型假体全髋关节置换术。观察2组患者手术时间、术中出血量及住院时间,术后1、3、6个月Harris髋关节评分(Harris评分)及术后总疼痛发生率、术后总并发症发生率。结果观察组患者手术时间明显短于对照组,术后1、3、6个月的Harris评分和术后3、6个月优良率均明显高于对照组,总疼痛发生率、术后总并发症发生率均明显低于对照组(P<0.05或P<0.01)。结论非骨水泥型假体全髋关节置换术治疗老年股骨颈骨折具有显著疗效,能够减轻患者的疼痛,降低并发症发生率,有助于改善患者的生活质量。

Objective To explore the clinical efficacy of non-cemented total hip arthroplasty in the treatment of femoral neck fractures in elderly patients.Methods A total 120 patients with femoral neck fractures were assigned to receive either non-cemented total hip arthroplasty（observation group,n=60）or cemented total hip arthroplasty（control group,n=60）.The operation time,intraoperative blood loss,hospital stay,Harris hip score,postoperative pain rate and incidence of postoperative complications were measured in both groups.Results Compared with control group,the operation time was shortened and the total postoperative pain rate and incidence of postoperative complications were decreased in observation group（P〈0.05 or P〈0.01）.In addition,the Harris hip scores measured at 1,3and 6 months after operation and the excellent and good rates observed at 3 and 6 months after operation in observation group were significantly higher than those in control group（P〈0.05 or P〈0.01）.Conclusion Non-cemented total hip arthroplasty can effectively relieve postoperative pain,reduce the incidence of complications and improve the quality of life in elderly patients with femoral neck fractures.
