妊娠期糖尿病患者半日门诊管理与效果分析

Study for the part-time outpatient management and effect analysis for patients with gestational diabetes mellitus

目的探讨妊娠期糖尿病半日门诊管理的措施以及效果。方法纳入我院2015年1月-2017年1月妊娠期糖尿病患者,抽取例数共计100例,按照随机等分原则将100例妊娠期糖尿病患者分成为对照组和观察组各50例。其中观察组患者予以半日门诊管理,对照未予以半日门诊管理,常规实施营养咨询门诊,对比分析两组患者相关知识掌握、门诊一次复诊、二次以上复诊、剖宫产、低出生体重儿、巨大儿、遵医行为、血糖控制情况,平均孕期增重以及各指标检查异常情况。结果观察组患者平均孕期增重低,疾病相关知识掌握评分、门诊一次复诊率、二次以上复诊率均比较高,血糖水平控制较好,剖宫产率、巨大儿以及低出生体重儿发生率低,遵医行为较好,组间数据比较差异有统计学意义(P<0.05)。结论对妊娠期糖尿病患者施予半日门诊管理,可强化患者对疾病、妊娠知识的认识,提高其自我管理能力,便于其良好遵医行为的构建和血糖的控制,降低母婴并发症的发生,值得临床推广应用。

Objective To study the method of half-day outpatient management and it's effect for patients with gestational diabetes.Methods 100 patients with gestational diabetes mellitus from January 2017 to January 2017 in our hospital was selected for the study.Patients was randomly divided into observation group and control group.50 cases in each group.Patients in observation group was given routine nutrition consultation and half-day outpatient management.Patients in control group was given routine nutrition consultation only.The related knowledge understanding,an outpatient visit,two or more outpatient visits,cesarean section,low birth weight and macrosomia,compliance,glycemic control,average weight gain during pregnancy and abnormal index for the two group was measured and compared.Result Compared with the control group,the average pregnancy weight gain in observation group is lower,disease related knowledge score,outpatient visit rate,more than two times the referral rates are relatively high,better control of blood sugar levels,cesarean section rate,fetal macrosomia and low birth weight infants with low incidence of compliance is better between groups.There was statistically significant difference between groups(P<0.05).Conclusion Management of gestational diabetes mellitus by half-day outpatient management can enhance patient understanding of the disease,pregnancy knowledge,improve their self-management skills to facilitate their good compliance behavior of building and hypoglycemic control,reduce infant and maternal complications.It is worthy of clinical application to reduce the complications of mother and infant.
